Job summary
This new post offers an exciting opportunity to lead the ongoing development of a progressive model of nursing care - the PATCH team "Providing Assessment and Treatment for Children at Home". The post holder will manage a cross-site team of nurses providing treatment to children and young people in their homes who would otherwise be admitted to acute inpatient wards.
Main duties of the job
The post holder will:
- Use their experience and skills in the independent assessment and management of acutely unwell children to ensure all children referred to the PATCH team receive high quality, effective, individualised care that is compassionate and caring.
- Provide clinical supervision and clinical training for the band 6 members of the PATCH team and other nurses that rotate through the team.
- Work closely with the lead paediatric ED consultants to oversee clinical governance and audit of the PATCH project and to lead, monitor and feedback on the project performance.
- Assist in the reduction of acute admissions to inpatient hospital beds by supporting children and families in the community to self-manage conditions
- To participate in auditing effectiveness and incorporate findings into practice.
- To ensure safeguarding issues are acted upon promptly and appropriately
