Job summary

We are looking for a motivated individual to provide clinic coordination for our HIV peer support service which covers 4 of Chelsea and Westminster Hospital Foundation Trust's HIV outpatient clinics, (Kobler Clinic,10 Hammersmith Broadway,56DS and Twickenham House) as well as Ron Johnson ward.

The role will involve working collaboratively with staff and services across clinical settings, enabling patients to access holistic and effective HIV Peer Support to improve their health and wellbeing.

Youwill be an integral member of the cross directorate HIV MDT and be being able to communicate the voice of our service users, and allowing this to inform decision making.

You will play a key role in linking in with external 3rd sector partnership work around Getting to Zero and Fast-Track Cities London.

Main duties of the job

To provide clinic coordination for HIV peer support across Chelsea and Westminster Hospital Foundation Trust's (CWHFT) HIV outpatients clinics and Ron Johnson Ward.

The role will involve working collaboratively with staff and services across clinical settings, enabling patients to access holistic and effective HIV Peer Support to improve their health and wellbeing.

The post holder will be an integral member of the cross directorate MDT, being able to communicate the voice of our service users, and allowing this to inform decision making.

They will play a key role in linking in with external 3rd sector partnership colleagues in the Fast-Track Cities Getting to Zero (GTZ) Collaborative

Clinical

Co-ordinate internal clinic referrals for people living with HIV(PLWH).

Work with project partners and cross directorate HIV MDTs where appropriate to ensure referred patients are linked directly into peer support.

Assist in re-engaging and supporting those patients who have disengaged from care

Providing a wellbeing assessment for new patients engaging in HIV care and those who are re-engaging in care. Working with patients to create a support plan to address any needs identified

Where necessary make relevant referrals to other CWHFT services, including any safeguarding, mental health or social care needs

Provision of direct casework support for people presenting with complex needs

Making external referrals to specialist agencies as appropriate.

Assigning patients with peer mentors, when appropriate.

Providing basic information on living well with HIV and its treatment

Supporting patients to effectively access healthcare services at CWHFT and externally, including GPs

Continual assessment of progress towards meeting clients needs

Work collaboratively within the MDT, as an integral part of all HIV clinics at CWHFT sites and with all the partnering organisations

Actively participate in individual and team supervision and staff and team meetings as required

  • Relevant qualification in Peer Mentor training/Project management
