Pain Advanced Clinical Practitioner, Band 8a
Cross site: Chelsea and Isleworth
We are looking for an enthusiastic advanced clinical practitioner to join our pain team as lead across our two hospital sites. The role entails working within our pain department as an advanced and autonomous practitioner.
The successful applicant will take a senior clinical role to deliver an innovative service within our pain team to deliver the service, support the Senior Nursing and Midwifery Team priorities and the wider Trust agenda. You will work collaboratively with the clinical leads while developing partnerships with the other divisions and other specialist nurses.
You will have had demonstrable success in a senior clinical role working as part of a multi-professional team, and bring strong advanced clinical skills. You will be able to establish your credibility with colleagues and be a visible senior nurse to an ambitious and high performing team. You will have extensive experience of caring for patients, good clinical and management skills along with well-developed ability to communicate and support colleagues, patients and their families throughout their care, treatment and follow up.You will be able to motivate and engage others to ensure that the services we provide supports the Trust's values, as well as having a pioneering approach that is not afraid to challenge convention and traditional ways of working.
Main duties of the job
This is a post for a fully qualified Pain Advanced Clinical Practitioner. The Pain Advanced Clinical Practitioner will be expected to uphold the four pillars of Advanced Practice, providing expert, evidence based care for patients, clinical leadership to assist and advise junior staff with decision making, and participate and innovate in education and research during their training. In doing so he/she will:
- Practice as an Pain Advanced Clinical Practitioner within own sphere of competence and agreed acceptable limits of practice
- Utilise advanced clinical skills to assess patients, order and interpret investigations, diagnose, treat, and refer patients with support and supervision from a senior doctor
- Be a pivotal part of the pain pathway including initial assessment, diagnostic elements and patient consultation
- Continue to develop, practice and expand own specialist skills
- To contribute to service improvement across Pain Department
- To contribute to the Workforce Strategy and Training and Development Programme for the Multidisciplinary Team
- To support and participate in all aspects of clinical governance, evidence based practice and quality assurance
- To ensure the principles of evidence based practice and adherence to current best practice guidelines are adopted across the service
- To contribute to an advanced practice workforce strategy for planned care service

Clinical Provision & Expert Practice
1. Provide nursing leadership and management of the pain service
- Maintain and provide nurse led out-patient pain services. This includes medication review clinics, Telephone follow up clinics, TENS and reviewing Spinal Cord stimulator patients
- Develop skills and knowledge to review new patients in the out-patient clinics.
- Provide expert advice on pain and symptom management to medical and surgical teams across the Trust.
- Co-ordinate the work of the specialist nursing team and in conjunction within the multi-disciplinary team, ensuring seamless continuity of care and delivery of service is maintained.
- Provide direct clinical support and guidance to pain specialist nurses in the management of their caseload and patients.
- Use physical assessment skills and analyse results and investigations as appropriate to aid diagnosis and treatment within areas of competency.
- Refer for diagnostic tests (i.e. X-Ray, MRI, ultrasound, EMG studies and blood tests) and demonstrate extended knowledge, for example, in the fields of: Radiography and other investigations.
- Facilitate and ensure that all pain management referrals are managed appropriately. This may involve appropriate triage, providing advice, assessing complex pain patients on the ward as part of an integrated referral pathway and assessing and managing patients in the outpatient setting to ensure timely assessment and intervention.
- Prescribe for acute and chronic patients and to provide expert advice on pain medication to other medical teams.
- Where required, to work in an extended clinical nurse specialist role administering advanced treatments such as Qutenza and on occasions the removal of Spinal cord stimulator devices.
- Provide management and support to the specialist pain nursing service
- Receive and make referrals to specialty teams and primary care
- To undertake non-medical prescribing in line with the Trusts non-medical prescribing policy.
Clinical Leadership/Management
- Provide management and support to the specialist pain nursing service on a day to day basis; this includes inpatient and outpatient pain.
- Be aware of and promote current developments in pain management, nursing, education and practice within the NHS.
- Be responsible for the development and implementation of appropriate protocols, guidelines and care pathways.
- Organise workload effectively, delegating effectively and supporting others in caseload management and practice.
- Lead on recruitment and retention initiatives within own specialty and to contribute to same within the Trust as required i.e. short listing and interviewing senior nursing staff.
- Undertake regular Performance and Development Reviews (PDRs) ensuring that specialist pain nurses have robust and achievable personal development plans.
- Encourage and promote evidence based practice and critical decision making in the Pain Department through role modeling and providing advice for members of the multi-disciplinary Pain Department team.
- Lead and assist in the collation of data for the service key performance metrics, and work with others to ensure continuous improvement on results.
- Develop links throughout the Trust through attendance at meetings and maintain communication networks across sites.
- Contribute to clinical effectiveness thought involvement in audit, risk management and clinical governance.
- Manage a safe clinical environment within the Health and Safety at Work act.
