Urology Pathway Transformation Manager

Fixed Term Contract: 12 months

An opportunity has arisen to join the Planned Care Surgery Operational team as a Urology Pathway Transformation Manager.

The purpose of this role is to support the Urology pathways in designing and implementing early diagnostic pathways for patients with suspected cancer. This role has been specially designed to support the Prostate pathway in the first instance and will require the post holder to evolve to support the pathway as it continues to refine over time. The post holder will work both directly and indirectly with patients and their carers to assist in coordinating the care of patients through their hospital care journey.

Main duties of the job

The post will require the coordination of appropriate services to maximise the delivery of the streamlined diagnostic pathways, thereby improving the quality and experience for patients and their careers, and diagnosing cancer earlier, specifically with a view to delivering 28 day faster diagnosis.

The post holder will support the management of the project as well as capacity and demand planning for rapid implementation across Outpatients, radiology and cancer.

This post is a fixed term post of 1 year with a responsibility to drive change within the service and set it up to be able to be manage as business as usual.

Job responsibilities

Main Duties and Responsibilities:

1. Manage the re-energisation of the prostate pathway cross site.

2. Coordinate prostate biopsy referrals and ensure all relevant information is present as per policy and protocols.

3.Ensure patient appointments are booked in line with the pathway protocols

4. To be a point of contact for GPs who have queries regarding their patients that have been referred to Chelsea and Westminster.

5. Coordinate the necessary assessments, appointments or investigations as identified

6. Monitor all aspects of care coordination and service delivery.

7. Act as the patient advocate and facilitator in order to resolve issues that may be perceived as barriers to accessing the service

8. Effectively utilise a computer and various software applications in order to enter and maintain patient data, communicate with the multidisciplinary team and complete other IT tasks as required by the post.

  1. To participate and display a willingness to learn, develop a relevant technical competency / area of interest and keep up to date with current national and local issues and developments.
  2. To routinely participate in departmental audits and as required under the supervision of managers and input data as requested.

  4. Lead on the collection of data for audit purposes and to monitor outcomes of pathway changes for RMP
