We are looking for an enthusiastic and innovative Trauma Nurse Coordinator who will help support our cohort of patient at the West Middlesex site.You will have individual, multiple skills and an appetite to deliver a first class nursing service at one of the UKs most successful and progressive NHS Foundation Trusts. The role entailspromoting excellence in nursing practice through the integration of expert clinical practice, education, consultancy, acting as a resource and commitment to nursing research. The successful candidate will work within a well-established team.

The Trauma Nurse Coordinator functions within a field of practice that focuses on the needs of the patient and encompasses interactions with others in delivering nursing, health and social care. The Trauma Nurse Coordinator also has a key role in the collection of data and planning of care for groups of patients within the orthopaedic pathways. This includes assessment, treatment and/or referral or discharge.

The key services delivered by this Division are Emergency and Elective Surgery, Theatres, Intensive Care and Burns services all of which are delivered through the busy but well organised and supported wards and departments.

Main duties of the job

The post holder will work in partnership with the multi-disciplinary team to effectively coordinate the management of trauma patients attending the Chelsea and Westminster and West Middlesex Site as in-patients and out- patients.

To work proactively and collaboratively with the NWL Trauma Network, resolving issues as they arise, upholding adherence to standards and protocols and by being an active member of the Clinical Advisory Group

Support the trauma data collection team from surgical/orthopaedics point of view by interpreting clinical information and assisting with analysis and presentation of trauma date. Liaise with senior clinicians to ensure all collated data are accurate and analysed.

The role will involve coordination of care of all trauma patients' adults and paediatrics, both pre and post-operatively along the length of their pathway.

Managing the Repatriations to and from the local Major Trauma centre.

The trauma coordinator must be able to prioritise and coordinate the management of such patients and communicate effectively with the patient and all member of the multi-disciplinary team.

The post-holder is also expected to help develop and maintain protocols for this group of patients and to be involved in data collection for the continued audit of this service.

Will possess highly developed specialist knowledge, underpinned by theory and experience.

Coordinate the patient journey of all emergency trauma cases through pre admission, diagnostics, treatment, rehabilitation and discharge ensuring effective management of this group of patients.

Ensure that no patient waits for theatre longer than is clinically necessary, trouble-shooting potential obstacles within the patient pathway to achieve this and escalating to senior management when assistance is required.

Ensure that all consenting patients with fragility hip fractures requiring surgery are operated on within 36 hours of admission to the hospital.

Use advanced communication skills to provide and receive highly complex and sensitive information regarding patients and the treatment they require, ensuring that patients and their relatives are involved in decisions about their care.

Be responsible for the care of patients awaiting surgery and maintain regular on going contact with them at home to provide support and advice until their surgery.

Be responsible to support patient on the Enhanced Recovery programme.

Liaise with theatres and ward/day surgery to ensure appropriate use of time and resource of trauma lists.

Compile daily trauma surgery theatre lists whilst taking into account urgency of different patients treatment and existing waiting times for admission for trauma.

Work in an unpredictable environment with regular unplanned emergency admissions, yet possess the ability to remain flexible and imaginative in their problem solving approach.

Develop escalation policy in collaboration with surgeons, managers & theatre managers when demand outstrips capacity for theatre.

Managerial

Develop mechanisms for monitoring regular patient feedback, using this to constantly develop and improve the quality of care, communication and service delivered to patients.

Assist in managing the service in the present time and developing its future course.

Provide professional leadership to the Virtual Fracture Clinic Specialist Nurse.

Liaise with the Major Trauma Centre as necessary for referrals in and out of that service to Chelsea & Westminster Hospital.

Contribute to, develop and maintain a communication system, which ensures effective dissemination of complex and sensitive information in relation to all aspects of patient care and service activity with patients, other units and the multidisciplinary team.

Work closely with Orthopaedic ACP for Trauma in maintaining and updating local (E-Trauma) and national database (NHFD)

Deal swiftly with any verbal or written complaints:

Interact with complainant to defuse the situation.

Investigate complaint and produce report as necessary.

Take appropriate action to ensure any deficiencies in the service are highlighted and in accordance with Trust Policy.
