Job summary
We are looking for two experienced registered nurses with experience in the specialty to join our Stroke services and to provide excellent inpatient and outpatient stroke/TIA care cross site. The post holder will provide clinical expertise both in inpatient and outpatient care, and will join a dynamic team based at WMUH and Chelsea & Westminster Hospital.
The post holder will improve the quality, effectiveness and efficiency of provision of stroke and TIA services for stroke patients. They will be responsible for facilitating rapid assessment of the patients and to co-ordinate the ongoing management of this patient group.
The CNS will co-ordinate, manage and develop pathways for the Stroke and TIA Services.
They will be required to ensure the Stroke Services are providing care to a high clinical standard by developing and implementing evidence based clinical guidelines for the management and rehabilitation of stroke.
Will work closely with the professional leads in order to provide the best service for stroke patients.
Main duties of the job
o To provide nurse leadership, in collaboration with the ward Sister/Charge Nurse, on a day to day basis to the Stroke Services' multi-disciplinary team and develop the service, ensuring the disciplines work together for the best interests of patients.
o On a daily basis, ensure the Stroke Services follow the policies and procedures, which are required to ensure the smooth and safe running of the service.
o To work with the multi-disciplinary team to ensure that operational systems are appropriate and responsive to patient needs.
o To attend Stroke Steering Group meetings and be responsible for the development and delivery of acute stroke services against national and locally agreed targets and standards.
o To attend relevant team and operational meetings.
o To develop and maintain an action plan of Stroke and TIA Services, including relevant audits in liaison with the multi-disciplinary team, professional leads and other service providers.
o To monitor progress and outcomes of service developments against action plan and standards.
o To ensure the Stroke and TIA service meets the services standards based on evidence of best practice and established systems for regular monitoring and updating.
o To practise as a professional nurse leader in a manner that supports and inspires colleagues - both within and without the Trust.
o To influence and lead changes in practice, service development and strategy.
