An exciting opportunity exists to join the Gastroenterology Team as a Band 7 Clinical Nurse Specialist. We are seeking an enthusiastic, forward thinking and motivated nurse to join this busy service. The nurse will have the opportunity to build upon and develop their knowledge of Inflammatory Bowel Disease, as well as being supported to gain formal qualifications such as speciality specific modules and non-medical prescribing.
Assist in the delivery of high quality Inflammatory Bowel Disease (IBD) services throughout the Trust.
The C&W hospital has a strong IBD service, looking after some 600 patients, led by a consultant and clinical nurse specialist team. The service is supported by a monthly paediatric/adult IBD transition clinic (in association with a Consultant Paediatrician) and a weekly multi-disciplinary IBD meeting.
A clinical and biological IBD database is under construction. Our clinical fellow is developing the national twin IBD registry, as part of an MD thesis.

Main duties of the job
Clinical
Perform comprehensive assessment of patient health needs in early detection of IBD
To provide a specialist nurse led service working autonomously in providing care for IBD patients and their families according to agreed protocols or as specified by the Consultant Gastroenterologists.
Plan, implement and evaluate care delivery according to changing health care needs
Perform advanced clinical skills in assessment &/or diagnosis &/or treatment
Involve patients and carers/relatives in the planning and delivery of care and development of services.
Monitor investigation results, initiating further assessment/review of treatment pathway as required. Monitor and manage patients whilst on treatment, including
Patients taking immunosuppressants and biologics. Explaining treatment goals and managing side effects
Work collaboratively with other professionals and agencies to ensure patient needs are met, especially in relation to on-going care needs and discharge arrangements
Establish and maintain effective communication with patients and carers/relatives, and professionals across health and social services and within IBD services
Contribute to the health promotion and education for patients, relatives and MDT and within the local health economy
