Job summary
A fantastic opportunity has arisen for a pharmacist to join the Rheumatology team based at the Chelsea and Westminster site in a Band 8a role.
The post will offer a range of exciting and unique clinical and professional development opportunities as you work within the Rheumatology and Pharmacy Department. You must be able to work as part of a multi-disciplinary team, to deliver excellent pharmaceutical care and services to our patients.
Main duties of the job
- To plan and provide clinical pharmacy services to meet the needs of rheumatology in accordance with the Medical staff and LDP Emergency and integrated care (EIC) in order to meet Trust objectives and the Clinical Governance Agenda.
- To plan the development and provision of pharmacist-led clinics for biologics and DMARDs in the field of Rheumatology alongside specialist consultants and nurses.
- To work with the Rheumatology MDT on improvement initiatives aimed at optimising medicines use in line with the key priorities
- To work independently to provide patient care ensuring effectiveness, safety and treatment and financial obligations are met
- To act as an 'independent prescriber' within Rheumatology and under supervision of lead Rheumatology clinician
- To support and supervise less experienced pharmacists, providing clinical pharmacy services for inpatients, within the directorate.

- To act as a clinical role model in the field of Rheumatology pharmacy and for inpatients within the directorate. Demonstrate the ability to provide safe, clinically effective and cost efficient use of drugs in these patients.
- Work as an independent prescriber in out-patient clinics/virtual reviews for patients on methotrexate, DMARDs and biologics, with appropriate monitoring to ensure safety
- Maintain boundaries of own practice within agreed guidelines and protocols, use professional judgment in managing complex and unpredictable situations and seek guidance from other relevant experts.
- Make appropriate referrals where necessary; ensure the clear and effective communication of patients needs to other health care professionals both within the trust and in other health care environments.
- Build an environment of trust and rapport, enabling patient empowerment in their healthcare and acting as an advocate for the patient in treatment decisions and choices.
- Work collaboratively with healthcare staff, managers, academics, improvers and patients to deliver the service development agenda for Rheumatology in accordance with agreed objectives
- Maintain accurate and contemporaneous records of patients assessment, care and treatment.
