CESR Rotations in Emergency Medicine

Our CESR programme at Chelsea & Westminster Hospital is an exciting opportunity intended to provide the clinician with the support and training required to successfully complete all aspects of the CESR process. It will be possible to provide specific 3 month secondment-style modules to address relevant educational and competency needs. Secondments include Anaesthesia, ITU and Children's Emergency Medicine. The remaining 9 months of the year will be working in our adult emergency department.

Our department is renowned for its training and support. The Consultant body has current FCEM examiners as well as experience at successfully becoming a Consultant via the CESR process supported by this department. The departmental lead of the CESR programme is heavily involved in the CESR training standards committee and thus has in depth understanding and knowledge of the processes and requirements involved.

There will be a specifically-designed training and teaching package for all successful applicants and there will be a designated Consultant supervisor for the duration of the rotation who will guide completion of the CESR portfolio.

These posts will participate in the 24 hour Emergency Department rota.

The rotation is initially offered on a fixed term contract for 12 months after which we would envisage extension on a mutually agreed basis, up to three years. The post is available for immediate start or a delayed start date may be discussed.

Main duties of the job

The ED has 9 resuscitation bays, two of which is dedicated to paediatric patients, a majors area with 21 cubicles and one theatre which is used for suturing and plastering. There is a separate Urgent Care Centre co-located within the emergency department supported by a team of Emergency Nurse Practitioners, and by GPs and ED doctors of all grades. We also have an 8 bedded emergency observation unit within the emergency department for patients who require a further period of observation (less than 24 hours) or input from the multidisciplinary rapid response team.

The Paediatric ED is adjacent to the adult Emergency Department and is staffed by a separate nursing and medical team, however we work closely and often help with cross-cover. The Department has a dedicated ED X-ray and CT scanner. We have very good access to advanced imaging like Ultrasound and MRI when needed.

Everyday Clinical Duties

The main service commitments of the CESR doctors will be, under the supervision of the Consultants, to supervise the FY2s/CT1s and the junior clinical fellows, while maintaining a 'hands on' approach with patients. Out of hours you will be expected to run the shop floor and manage the department.

The department has initiated new patterns of working to achieve the current targets and integral within these are the resus lead and rapid assessment team which are roles which will be undertaken by the CESR doctors within the department. The CESR doctors would also be expected to help run the daily review clinic. The Consultants are very committed to teaching and post-graduate training and will ensure appropriate opportunities for professional development.

The rota is a full shift pattern providing 24 hour cover to the Emergency Department with 1:3 weekends. The shift pattern is new deal compliant with a 1A banding.

The pay banding may change to comply with new working practices or regulations relating to the reduction of Junior Doctors Hours.

POST GRADUATE EDUCATION

Our department is renowned for its training and support. The Consultant body has current FRCEM examiners as well as experience at successfully becoming a Consultant via the CESR process supported by this department. There will be a specifically-designed CESR training and teaching package for all successful applicants and there will be a designated Consultant supervisor for the duration of the rotation who will guide completion of the CESR dossier.

Regular time tabled teaching sessions take place and the CESR doctors will have protected time to attend this teaching.

Our EM teaching sessions are well attended and positively reviewed.

Medical students are regularly attached to the Emergency Department for four week placements. The successful applicants will be regularly involved in teaching activities in the department and the hospital.

Study leave

Successful candidates are entitled to study leave which will be negotiated with their educational supervisor.
