Job summary
We are seeking a 6 PA Consultant in Palliative Medicine to join our team here at Chelsea and Westminster Hospital NHS Foundation Trust at the West Middlesex site. This is a replacement substantive post.
The West Middlesex site has a long history of pioneering, innovative healthcare. Based in Isleworth it is at the heart of the local community- a modern, award-winning hospital with state of the art facilities.
The successful applicant for this post will share responsibility for providing medical consultant leadership for specialist palliative care (SPC) at the West Middlesex University Hospital (WMUH). At the West Middlesex site, the SPC team comprises 5 full time Clinical Nurse Specialists, a SPC Consultant (0.8WTE), a SPC Clinical Fellow, a cross-site Lead Nurse for SPC and End of Life (EOL) Care (1.0 WTE), a SPC Occupational Therapist (0.4WTE) and secretarial support from a Team Assisstant. This post offers an exciting opportunity to shape further development of the specialist palliative and end of life care services at the West Middlesex site of Chelsea and Westminster NHS Foundation Trust. Main duties of the job
To ensure active clinical communication between specialist palliative care services within the locality and with regional centres such as Imperial College Healthcare Trust and the Royal Marsden Hospital.
To actively participate in the delivery of the Trust wide End of Life education programme, including utilising the new 'Life Cast' manikins for SIM education and training.
To provide clinical supervision to a Specialist Palliative Care Clinical Fellow and a GP ITP.
To participate in the regular Palliative Care MDT meetings and other site specific MDT meetings as appropriate.
To support the 7 day a week face to face service for end of life and specialist palliative care.

The post will be based at WM, with the flexibility to work across site as necessary. The key aspects of the post will be:
- Medical support to the in-hospital Clinical Nurse Specialist Team for Specialist Palliative Care (SPC)
- Ensuring provision of face to face SPC expertise to patients and colleagues across the hospital site
- With the SPC clinical lead at CWH, supporting the development of the multi-professional specialist palliative care team at the WM site.
- SPC consultant input to the weekly Lung Oncology clinic to support symptom control, emotional support, information needs and onward referral to community SPC services
- Development and utilisation of relevant data collection tools, outcome measures and key performance indicators, supported by audit, research and service review
- Collaboration with colleagues at Chelsea and Westminster NHS Trust to establish joint working practices across, and relevant to, both sites
- Commitment to education and training initiatives in both specialist palliative and end of life care in all care settings, and to all stakeholder groups
- Leading on the development and delivery of the education strategy for the Trust SPC team
- To engage in all aspects of Good Clinical Practice including Teaching, Audit and Clinical Governance.
- Support the nurse led 7 day visiting service for SPC in the hospital. There is no requirement to work weekends in this post. The postholder will provide support such as planning for patients who need to be seen over the weekend and supporting the continued professional development of staff working at weekends.
