Job summary

An exciting opportunity has arisen for a Diabetes Specialist Nurse (DSN) to be at the forefront of driving up standards and quality in diabetes care in Community and Primary Care Services across the city of Liverpool. The role will include working in a variety of other clinical areas, local communities and the Primary Care Networks. Working in partnership with the diabetes teams of Liverpool Universities NHS Hospital Trust and services delivered by Mersey Care the role is diverse, varied and offers the opportunity for innovation and change. The post holder will be a non-medical prescriber and have experience of working autonomously within the field of Diabetes and with the ability to work to evidence-based guidelines; audit the care given to patients to make and suggest improvements in services. The post holder will also be required to deliver complex educational packages to health care professionals and patients around effective diabetes management.

Main duties of the job

Supported by the Team Leader the post holder will provide clinical expertise and professional resource to Multi-Professional teams, patients and carers within the Speciality service. The post holder will support the development of specialist area of practice in partnership with other members of the speciality teams across Acute, Community and Primary Care Service. The post holder will be a major contributer to Diast Team working on a defined case load offering highly specialist advice, setting standards of care, the assessment of care needs and the development, implementation and evaluation of programmes of care. The post holder will contribute to achieving the objectives of the clinical service group/speciality corporate objectives and work within the clinical governance framework. As part of the Community Diabetes Team the post holder will support the development and delivery of a programme of practice and professional development for delivery of care within the specialist area across the Liverpool Health Economy.

The role of the band 6 CNS in Diabetes includes the following: -

  • Perform specialist assessment of people with diabetes. Plan, implement and evaluate care delivery using advanced clinical knowledge and clinical skills.
  • Undertake the diabetes competency programme of education based on the national TREND Diabetes competencies.
  • Lead clinical care by managing a patient caseload providing expert assessment, planning and evaluation with the support of the band 7 CNS and the wider MDT.
  • Co-ordinate and manage patient reviews from the Diabetes Dashboard or other referral systems.
  • Contribute towards a multi-professional approach in the management of patients within the specialist area, especially in relation to ongoing care needs and discharge arrangements.
  • Support patients and their families and ensure that they receive the required information to enable them to self-manage, lead or participate in their care delivery and to support safe discharge.
  • To support, assist and encourage the multi-disciplinary team in the review of people with complex needs, by the provision of new and innovative models of case management.
  • Takes responsibility for own continuing professional development and performance, including identifying own development needs and maintain own portfolio in accordance with re-registration requirements.
  • Participates in the supervision and the teaching of pre and post registration students, new staff members and junior medical staff. Providing in-reach education to clinical areas to support safe diabetes patient care.
