Job summary

We are looking for an enthusiastic community diabetes specialist nurse (CDSN) to join our community Diabetes multidisciplinary team.The role will work closely with the SSMTR ( Supplementary Substance Misuse, Treatment and Recovery) Liverpool project to provide the delivery and development of a multi-modal clinic in the four drug and alcohol centres within Liverpool Place.

The successful applicant must have prior experience in caring for people with diabetes, in a community setting. You must be self-motivating, organised, computer literate and able to demonstrate excellent interpersonal skills.

Together with the other members of the multidisciplinary team, the post will provide diabetes care to patients with specialist diabetes needs, facilitating a smooth and safe journey for this cohort of people. Main duties of the job

The post holder will work closely with the SSMTR ( Supplementary Substance Misuse, Treatment and Recovery) Liverpool Project and the community teams for Respiratory, Liver, Cardiovascular and Kidney Health Teams.

The rolewill act as the clinical expert and professional resource to the Multi-Professional team, patients and carers within the project.

The post holder will take overall responsibility to develop the specialist area of practice in partnership with other members of the speciality team working collaboratively to develop services and quality of care delivered.

The post holder will carry responsibility for a defined case load, and this will include setting standards of care, the assessment of care needs and the development, implementation and evaluation of programmes of care.
