** Previous applicants need not apply** An exciting opportunity has arisen in our department of Medicine for Older People, Endocrinology and Diabetes for a Patient Pathway Co-ordinator. This is a varied role that will see the appointed candidate working to provide an efficient administrative service to take an active part in ensuring waiting lists are managed in accordance with Trust polices and procedures. We're looking for an enthusiastic, motivated candidate that can demonstrate the ability to meet deadlines, multitask, plan their own work load and work well as part of a team. The role will see the successful candidate taking a lead role on managing RTT,booking patient follow-up appointments from a waiting listand supporting the department in an administrative capacity. Main duties of the job
The post holder's main duties will include:
- Ensuring patients are allocated and booked to the correct clinics/waiting lists following the patient's consultation. This may involve meeting and discussing patient outcomes with Consultant medical staff
- Liaise with the specialty and operational teams to ensure clinics are cancelled, when appropriate, in accordance with the patient access policy.
- If required, notify patients of appointment dates and any cancellations and reschedule as necessary.
- Monitor waiting lists to ensure compliance with local and national policies and procedures
- Escalate areas of concern to ensure our patients receive the right care at the right time
- Work closely with our patient access centre to ensure continuous improvement in the outpatient service
