Job summary

The post holder will play a key role in the Acute Oncology Team (AOT) & in the delivery of acute oncology services throughout the Trust. The post holder will provide early input of specialist care to oncology patients enabling

  • Faster and better care of patients with complications of Chemotherapy (early recognition, better treatment, early discharge)
  • Faster and better care of patients with complications of cancer (early recognition, better treatment, rapid referral back to specialist Oncology Team, Palliative Care Team etc
  • Appropriate investigation of patients who might benefit from treatment for unknown primary cancers (targeted investigations, rapid triage of patients into specialist oncology or palliative care).

The post holder will give advice and implement patient care in an area which requires specific clinical knowledge, skills and experience. The post holder will work alongside other clinical colleagues in Oncology, Palliative care, ED, Acute Medicine, Surgical, Radiology and Pathology to ensure quality of care to the patients is maximised. This will include the development of clinical guidelines & education programmes

The post holder will carry responsibility for either a defined case load or offering "highly specialist service advice" this will include setting standards of care, the assessment of care needs and the development, implementation and evaluation of programmes of care.

Main duties of the job

. Triage and perform specialist assessment of patients nursing needs, plan, implement and evaluate care delivery using advanced clinical knowledge and clinical skills.

Contribute towards a multi-professional approach in the management of patients within the specialist area, especially in relation to ongoing care needs and discharge arrangements.

Support patients and their families and ensure that they receive the required information to enable them to self-manage, lead or participate in their care delivery.

Recognise changes in patient's condition that require the intervention of others and highlight this as part of the supportive Advisory and Liaison role the AO Team provides.
