Job summary

If you are a highly motivated person with stroke experience, you will not want to miss out on this exciting opportunity to join the Early Supported Discharge Team (ESDT) for stroke as a Band 4 Assistant Practioner working in the Liverpool Stroke Recovery Partnership (LSRP). We are looking to recruit a dynamic individual with a passion for high standards of care for our patients.

The LSRP is an integrated ESD service provided by Liverpool University Hospitals. This innovative team provides an equitable service across the city for stroke survivors and their families.

You will manage a caseload with the support of registered staff, assessing and developing treatment programs for Stroke patients, which will include patients with Occupational therapy, Physiotherapy and Speech and Language therapy needs. Patients are seen in the most appropriate environment to meet their need which is usually in the community.

We are looking for someone who has experience working with stroke/neurological patients. You will be well supported by a highly experienced multidisciplinary team that includes OT's, Physio's, Psychologists, Nurse, Speech Therapist, and therapy assistants.

Annex 21 Training Posts will be considered with support provided towards Foundation degree relevant to therapies (if required).

You will be required to have a full driving licence and access to a vehicle.

Main duties of the job

-To undertake skilled support work independently, as identified by qualified Therapy staff.-To carry own caseload-To carry out stroke specific rehab using set treatment protocols across 3 disciplines; Physiotherapy, Occupational Therapy, Speech and LanguageTherapy, monitoring progression and making minor alterations as required.-Work both independently and within the multidisciplinary team setting, reporting back patient progress and informing Therapists of issues or concerns.-To be responsible for specific tasks within the overall treatment programme and care for patient, e.g. carry out treatment programmes, arrange the supply of assistive equipment, liaise with outside agencies such as GP's and social work team.

Offers of employment may be made for a band 4 training post under the Annex 21 AFC terms and conditions. As part of this training post we would support the successful candidate with the completion of a Foundation degree relevant to therapies (if required).

To be accountable for own clinical practice whilst undertaking the assessment and treatment/intervention of identified patient group within agreed parameters and defined protocols of care.

To participate in and contribute to audit and service development

Carry out stroke specific rehab care with patients in a manner that respects their patient centred goals, abilities, privacy, dignity and individuality.

Manage the rehabilitation of patients as delegated by the qualified Therapist, working without direct supervision and responsible for assessing the progress of patients,and reporting back to the MDT.

To liaise regularly with the qualified therapist regarding treatment progression and ongoing care. Reporting to Therapist and the multidisciplinary team any lack of progression or negative changes in patients ability to undertake treatment session.

To undertake Home Visits/treatments at home considering lone working policies and risk assessments

To assess patients and carers safety in use and suitability of equipment within patients home environment and advise on environmental hazards in patients

Ensure equipment is clean and fit for its purpose for individual treatment sessions.

To travel between patients homes independently and adhering to infection control and PPE policies.
