Job summary

The family peer support workers will offer direct support for families and carers of individuals with a diagnosis of autism and/or a learning disabilities who are receiving support via the keyworker service.

The team offers support across all 4 areas of the Black Country Healthcare Foundation Trust (Wolverhampton, Dudley, Walsall and Sandwell) . The family peer support worker will be a time limited offer working directly with families and carers.

The Learning Disability and Autism Commissioning, Case Management and Keyworker Team are seeking Family Peer Support Workers to work alongside the keyworker team. The Keyworkers are part of the NHS long term plan. The vision is for all children and young people with learning disabilities and/or autism to have a designated key worker by end 2023/24. Initially the priority will be for all children and young people currently in hospital to have a key worker, then for children and young people who are at risk of going into hospital. (red/amber on the Dynamic Support Registers), and with evaluation of the pilot, the aim will be to develop keyworking for all children and young people with learning disabilities and/or autism by end 2023/24.

To have a pro-active approach, empowering families to identify their own needs.

To have an awareness of local provisions and support with signposting.

To be part of the key working team within the Black Country Healthcare Foundation Trust.

Have knowledge and understanding of the impact supporting a child and/or young person with a learning disability and/or autism can have on a family unit. This can be from a personal or professional perspective.

To undertake any other duties of a similar nature consistent with the responsibilities of this post in order to provide a quality Keyworker service.
