Job responsibilities

1. Professional

1.1To adopt and co-ordinate a multi-disciplinary agency approach to care, ensuring that all aspects of the patients needs are met.

1.2To set professional standards of care and to monitor, measure and audit the effectiveness of care given.

1.3To ensure an effective communication mechanism is in place, particularly with regard to patients care and progress, to include patients and relatives.

1.4To deploy, supervise and support other members of the nursing team in the provision of a professional nursing service.

1.5. The utilisation of available resources and services within the community setting, identifying any inadequacies and taking appropriate action to enhance the quality of care available within their client group.

1.6. To provide advice and education on the promotion of mental health and the prevention of illness.

1.7To adopt a research approach to nursing and utilise all relevant research findings.

1.8To act as a duty worker and manage assessment and crisis work accordingly.

1. Administrative

2.1. To ensure that all policies, procedures and guidelines are followed, reporting any deviations to senior staff.

2.2. Maintains all appropriate records as required by the Trust and Wellbeing Team and that accurate nursing records are maintained.

2.3. Ensure that there is efficient transfer of clients between team members and accordance with local policy.

2.4. To provide statistical returns as required by the Trust and Wellbeing Team Manager.

2.5. To attend and participate in clinical/departmental and team meetings as required by the service.

2.6. In accordance with local policy and procedure to utilise information technology.

2.7. To be involved in the selection and recruitment of staff in conjunction with senior staff as appropriate.

2.8. To plan and participate in the induction of new staff.

Promote Positive Team Culture and Team Working

To provide leadership as an effective member of the multidisciplinary team through:

Ensuring that all staff are aware of and understand CPA framework and how this is delivered within the team.

Staff attendance at CPA reviews and other whole team meetings.

Active collaboration with wider multidisciplinary team members to ensure wider participation in CPA care delivery.

Providing strong and effective leadership in partnership with the medical team and other lead multidisciplinary clinicians in formulating treatment plans.

Communicate with all necessary external agencies in supporting service users treatment plans and outcome measures.

Support and facilitate all external agencies in carrying out the assessment process enabling funding agreements.

Support Team Managers in their overall responsibility for the personal development and education of team members, including in-service training.

Promote Positive Mental Health Practice

To ensure that the clinical team work to an agreed model of mental health practice that supports the Recovery principles and promotes hope and optimism for all service users and their carers (where applicable).

Promote active patient involvement and carer involvement the development, delivery and evaluation of mental health practice initiatives.

Ensure that patients receive a high standard of care according to their individual needs.

Ensure that the team understand their role and responsibility in respect to wider national mental health policy and guidelines.

Actively seek out and implement evidence based practices.

In collaboration with the MDT promote, implement and evaluate specific outcome measures for all patients.

Assist in ensuring clinical audit becomes part of everyday practice

Monitor patient records to ensure they are up to date and are an accurate reflection of the situation.

Education/Qualifications

Essential

• oRMN
• oEvidence of Post Registration training
• oTeaching and assessing

Experience

Essential

• oPost qualification experience of working in mental health.
• oAble to conduct detailed assessment of a persons needs to include mental health, risk and its management, personality issues people presenting with a learning disability and other potential vulnerabilities
• o Acquired sufficient additional post-registration knowledge through experience as a nurse in a mental health setting to be able to work autonomously in the community.
• oExperience of working in a multi-disciplinary team.
• o Additional knowledge acquired through formal or informal specialist training and experience in order to be able to manage a caseload of clients with complex needs.
• o Direct work with mental health service users and carers.
• oWorking with statutory and voluntary agencies.
• oProviding support /supervision to other professionals
• oAbility to work under pressure to agreed deadlines and quality standards
• oSubstantial experience of working with people with acute mental difficulties within area of speciality
• oDemonstrate an understanding of CBT interventions, PTSD and Personality Disorder
