An exciting opportunity has arisen within the Memory Assessment Service in Sandwell based at Edward Street Hospital. The service operates 08:00hrs to 16:00hrs Monday to Friday. We are currently looking to recruit an experienced NMC Registered Mental Health nurse or registered Nurse with previous experience working with adults or older adults with mental health needs or dementia to work with the Memory Assessment Service on a Full time permanent contract. Part time or consideration for a secondment post is also available for consideration. Incentive for flexible working considerations.
The successful applicant will be responsible for the assessment, treatment and planning of care for patients who may have memory difficulties. Theywill need to be an enthusiastic and committed individual and will be part of a fully integrated Multi-Disciplinary Team comprising of RMNs, Clinical Psychologists and psychiatrist Consultants.

Main duties of the job
Main Duties/Responsibilities
The post holder will have the following remit:
Will be working providing care and support for a defined group of patients (caseload) who have been accepted for an assessment by the Memory Assessment Service.
Under the supervision of the Team Manager, arrange and organise appointments for service users of the Memory Assessment Service, and conduct assessments and reviews of treatment with cognitive enhancing medication, cognitive functioning and other relevant service user and carer related issues, both in the home, clinic and residential care settings.
- The post holder will be responsible for the smooth, overall day-day running of the service, ensuring policies and clinical procedures are relevant to working practices, maintenance of a safe environment, supervision, mentorship and participation in appraisal process
- Work in partnership with the multi-disciplinary team and contribute to the continuing development of the service.
- To facilitate access to relevant services in the statutory and non-statutory sector.

- To take responsibility for day-to-day management / running of the Memory Assessment Service in the absence of the Team leader.
