Job summary

We have newly created clinical divisions and we're currently developing new models of integrated primary and secondary care for adults and older adults, as well launching a far-reaching inpatient strategy, working to eradicate dormitories and building new facilities.
This ambitious model is multi-agency and multi-disciplinary, opening the door for you to work in a collaborative and flexible way with partners, service users, carers and their families.
Taking on one of our community roles, you'll play a key part in preventing and reducing the likelihood of admission, and improve the flow of patients from the community into secondary care.

Main duties of the job
As part of our Community Transformation Programme and exciting opportunity has arisen for a band 6 In Reach to Care Homes Worker to support the further development of our In reach to Care Homes Service. These posts will form part of our Enhanced Community Mental Health Team for Older Adults and will work alongside other Multi-Disciplinary Health professionals in delivering care to patients in the care home.
The aim of this service is to ensure that care home patients have an improved experience through high quality care and access to health professionals with the skills to support their Mental Health needs
