Black Country Healthcare NHS Foundation Trust, DudleyBand 5 Community Staff Nurse (Health Visiting)Would you like to gain experience and develop your potential within the Health Visiting Service?We are able to offer a development role to an enthusiastic Band 5 Registered Nurse on Part 1 of the NMC register (Midwifery, Child, Adult, Mental Health or Learning Disability branch) who have a genuine interest in the health and wellbeing of Children and Families. You will work within a skill mixed Health Visiting Community Team in the Dudley Borough.The post holders will need to have, and be able to show written evidence of Maths and English at GCSE level at C or equivalent, and haveevidence that they have achieved a Degree in a relevant area of health and social care, along with the relevant skills and experience to supportus in providing a place where we can achieve healthier, happy lives.There are 2 posts available across Dudley (Halesowen and Stourbridge)

Main duties of the job

There are five Health Visiting teams in various locations within Dudley who are all high achieving, supportive and dynamic and they are looking to recruit a Band 5 Nurse Development Post to join their existing teams.Prospective candidates do not need previous experience of community working, but will expected to have a passion for making a difference to the lives of pre-school children and their families. You will need to havean understanding of the public health role in order to support the delivery of the 0-5 years Healthy Child Programme.The successful candidates will need to be flexible team players who are able to demonstrate a willingness to support and implement change. You will be required to work in partnership with children and families inaddition to other agencies and multi-disciplinary teams, to support the delivery of evidenced based programs of care, in line with the Healthy Child Programme.

About us

In return, once a set of competencies are met in practice, it is expected that you will further develop and qualify as a Health Visitor via the Apprenticeship route, by completing the Health Visiting course at theUniversity of Wolverhampton (post-graduate level), subject to the availability of places.Access to own transport and the ability to travel is essential. You may be required to work across teams to ensure service delivery is met.The posts available are full time post (1.0 WTE / 37.5 hours a week)

Job PurposeThe post-holder will work as a member of the Health Visiting Service, working in partnership with families to deliver appropriately delegatedelements of the Healthy Child Programme (Department of Health 2009). The individual will support the delivery of evidence based programmes ofcare in order to provide a quality service which aims to improve health outcomes for children aged 0-19 years.Clinical Responsibilities To undertake Healthy Child Programme developmental assessments as directed by the named or delegating Health Visitor, ensuringprompt feedback and escalation of concerns as required. To identify health needs of children and families and in agreement with the Health Visitor plan, deliver and review appropriate evidence basedinterventions in line with the Standard Operating Procedures and Pathways to improve health outcomes. To support families through challenging situations such as illness or bereavement under the direction of the Health Visitor. To follow up Accident & Emergency attendances offering appropriate advice and support to families. To participate in the planning, delivery and evaluation of evidence-based health promotion activities for individuals, families andcommunities. To follow local and national policies and procedures regarding safeguarding children and vulnerable adults, identifying families withsafeguarding concerns and escalating to the Health Visitor to agree a plan of action. To support the Health Visiting Team in delivering Early Help Interventions and to participate in multi-agency working processes such asShortlisting packTeam Around the Family (TAF) meetings under the supervision of a health visitor. To participate in and promote the childhood immunisation programme. To promote breastfeeding in line with BCHCT Policy and to participate in BFI Audits and Practical Skills Assessments as required. To support families with children who have been identified as having additional needs, contributing to the completion of Education andHealth Care Plans and supporting families with the transition to school. To obtain service user feedback in order to ensure that services are developed to meet the need of clients. To establish and maintain partnerships with families and other agencies eg GPs, Midwives, Childrens Centre Staff, Early Yearssettings to ensure effective communication and co-ordination of care. To ensure clinically effective working relationships by working positively and communicating effectively with members of the HealthVisiting Team. To support students in clinical placements within the Health Visiting Team. To work flexibly within BCHCT in order to meet demands placed on the service.Professional Responsibilities To be accountable for own practice, recognising own professional boundaries, and seeking advice and support when necessary. To be responsible for own personal and professional development and to develop competencies in Community Staff NurseCompetency document. To participate in annual staff appraisal and agree objectives in line with BCHCT key priorities.
