Would you like to be part of shaping our Intensive Support Team for adults with Learning Disabilities and to develop your specialism in this area?

We have a real opportunity to shape an integrated MDT with dedicated Speech and Language Therapy provision.

The post holder will be working within a Learning Disability specialist Intensive Support team supporting patients across the wider Black Country area. The team is part of the Transforming Care Assessment and Treatment pathway with a focus on avoiding hospital admissions.

We are looking for an enthusiastic therapist who is looking to develop into this area of specialism. We have an established competency framework for transition from Agenda for Change band 6 into band 7. There is excellent management and supervision support and connection with MDT and SLT colleagues.

Main duties of the job

This post involves providing specialist Speech and Language Therapy assessment and intervention to adults with learning disabilities presenting with a broad range of communication difficulties and delivering and developing training for Trust staff, care providers and families.

We are a forward-thinking, friendly and supportive specialist Speech and Language Therapy service. We have 18 Speech and Language Therapy staff working across the service within the community, enhanced inpatient settings and pathway teams reducing hospital admissions across the Black Country.

Continuing professional development is actively supported via annual appraisals, opportunities to attend webinars/courses, regular clinical supervision opportunities and through networking and skill sharing with SLT colleagues from across the service.

Job description

Job responsibilities

Assesses, diagnoses and manages the communication and/or swallowing needs of clients, in particular those with the most complex difficulties, including those with challenging behaviour, high acuity and mental or physical health concerns, to reduce the impact on quality of life and reduce hospital admissions.

Ensures timely SLT response according to service standards to referrals to the IST and Inpatient service.

Contributes clinically as part of the intensive support pathway to both inpatient and intensive support team settings (community and inpatient) to improve health and wellbeing outcomes and reduce hospital admissions.

Assists service leads to develop the intensive support pathway providing advice to the MDT, Unit Manager, Pathway Leads and Professional Lead as relevant in relation to clinical role.

As part of an MDT approach, facilitates smooth transitions and effective working between clinical pathway services to proactively improve the patient experience.

Provides both direct and indirect intervention regarding both communication needs and/or dysphagia in various forms including assessment, therapy, consultation, advice, staff training and facilitation; working within an inter-disciplinary model in order to ensure that the inter-agency needs of clients are met.

Provides clinical supervision and professional development to more junior SLT clinical staff and supports AHP staff as an integrated member of the inpatient, IST team and community pathway.

Undertakes training and skills development associated with specialist role including recording continuous professional development and taking part in regular management and clinical supervision.

Takes an active role in clinical multi-disciplinary formulation where speech and language therapy recommendations contribute to the holistic management of a patient to optimise therapeutic management and meet pathway aims.

Responsible for managing his/her own caseload including team and clinical outcome measures as part of the Intensive Support Team and inpatient services.

Contributes to divisional SLT service development, including speech and language therapy service audit, research and evaluation in clinical area.
