Job summary

The aim of the post is to support the Team Leader in leading the Children's learning Disability provision within Old Park Special School and Pens Meadow Special School (including post 16 provision) and in the community in the Dudley Area. The post holder will work with other members of the team to ensure that interventions and clinical procedures are provided and develop strategies to support this.

Main duties of the job

  • Providing learning disability support for children and young people for severe learning disabilities in Dudley who attend either Old park Special School or Pens Meadow school and report to the team leader.
  • Manage the assessment, planning and implementation of care for children and young people up to the age of 19 years , with a diagnosed severe learning disability.
  • Manage and oversee the Implementation of interventions to promote the health and wellbeing of children, young people and families affected by severe learning disabilities

Job description

Job responsibilities

  • Work within the Integrated Childrens Community Nursing Team as the band 6 Special School Learning Disability Nurse providing learning disability support for children and young people for severe learning disabilities in Dudley who attend either Old park Special School or Pens Meadow school and report to the team leader.
  • Manage the assessment, planning and implementation of care for children and young people up to the age of 19 years , with a diagnosed severe learning disability.
  • Manage and oversee the Implementation of interventions to promote the health and wellbeing of children, young people and families affected by severe learning disabilities

  • Continence difficulties
  • Behavior management
  • Epilepsy
  • Dietary advice
  • Emotional health and well being
  • Health management
  • Work as part of an integral Multi-disciplinary Team maintaining responsibility for a designated case-load acting with autonomy as appropriate
  • Work collaboratively with school staff including providing EHCP reports and attending EHCP/Team Around the Family meetings.
  • To positively represent the needs and rights of children and young people with severe learning disabilities, promoting equality and assisting access to NHS and any other services
  • To provide skills and strategies for young people/parents and carers to effectively manage their own/their childs condition
  • To maintain excellent working relationships between universal and specialist services,
  • To work as an autonomous practitioner, regularly working alone and unsupervised in a variety of different settings, organizing and managing own time and case-load
  • To provide advice, consultation and information to others on Learning disability issues, to ensure that the health needs of individual children and young people with learning disability are met appropriately
  • To provide reports as required on the provision of the Childrens Learning disability nurses
  • Working in conjunction with other agencies, ensure the appropriate planning for the transition of care takes place in a timely fashion and meets the ongoing needs of the

child/ young person and family.

  • To work with Adult Learning Disability partners to implement a smooth transition form paediatric to adult services
  • To communicate and act as a resource with relatives and/or children, lead professionals/ education regarding planning, implementing and reviewing care.
  • Ensure that any KPI;s, patient stories, contacts data are produced and delivered in a timely manner- working with the team leader to address any issues.
  • Liaison with, for example, GPs, AHPs and hospital paediatricians
  • Complete clinical nursing interventions with the child and family and ensure own competencies are maintained and provide any training as required to support independence. This will include:
  • Enteral feeding
  • Suctioning
  • Management of epilepsy
  • Medication administration
  • Oxygen administration
  • Providing independent clinical nursing care and support to special schools under direction of the team leader
  • Other nursing skills as deemed appropriate
  • To work within and contribute to, the Guidelines for Management of Medical Needs in Schools document.
  • To support the development of care pathways regarding the management of complex health conditions for children and young people within Dudley.
  • Work in conjunction with the other nurses in the integrated team and provide cover as required depending on scope of practice
  • To undertake any other duties of a similar nature consistent with the responsibilities of this post in order to provide a quality service.
  • To line manage members of staff, ensuring that any absence management/clinical supervision and appraisals are completed.
  • To be aware of budget constraints and working in conjunction with the band 7 devise ways that measures can be addressed.
  • To teach other qualified and unqualified staff.
