Job summary

To provide a specialist memory assessment service to people who experience memory problems, their families and carers within community and clinic settings.

To be responsible for effective assessment of care needs, planning and implementation of care plans ensuring a positive service user experience.

To provide specialist advice and education to other services and professionals.

To take a lead role in the continuing development of the Memory Assessment Service through research and clinical audit.

Main duties of the job

Will be working providing care and support for a defined group of patients (caseload) who have been accepted for an assessment by the Memory Assessment Service.

  • Under the supervision of the Team Manager, arrange and organise appointments for service users of the Memory Assessment Service, and conduct assessments and reviews of treatment with cognitive enhancing medication, cognitive functioning and other relevant service user and carer related issues, both in the home, clinic and residential care settings. Both verbal and written liaison to the appropriate person or agency will be required.
  • The post holder will be responsible for the smooth, overall day-day running of the service, ensuring policies and clinical procedures are relevant to working practices, maintenance of a safe environment, supervision, mentorship and participation in appraisal process
  • Work in partnership with the multi-disciplinary team and contribute to the continuing development of the service.
  • To facilitate access to relevant services in the statutory and non-statutory sector.
  • To participate in any duty system
