A great opportunity has arisen to work alongside the Discharge Team at Worthing Hospital as a permanent Fast track discharge co-ordinator .
Experience of end of life care and competence in completing the Fastrack Tool is required for this role.

This role is eligible for visa sponsorship under the UK Visas & Immigration (UKVI) Skilled Worker route however sponsorship can only be provided where applicants meet the specific requirements set by UKVI, including the relevant skill and salary thresholds (£25000), and any role-specific criteria.
Main duties of the job
Act as key-worker to co-ordinate individual complex discharges- with Main focus on End of life and Fastrack completion.
Responsible for co-ordinating rapid discharge for patients eligible for Fast track continuing health care funding.
Ensure compliance with NSF for NHS Continuing Healthcare and act as the Trust's liaison with NHS Continuing Healthcare.
Act as Trust expert in discharge planning, assisting and supporting the MDT to drive and co-ordinate discharges, ensuring effective, timely intervention from all disciplines
Assist wards and teams with understanding and using the 'Home First' Model
Support the Tranger of Care Hub in matching patients to pathways
Provide education in End of Life discharge planning to members of the MDT or other professional groups requiring this teaching
Support transfers to NHS funded community venues/services
Monitor performance associated with discharge, identifying gaps in service provision that may have a detrimental effect on length of stay, delayed transfers of care or patient experience
Work with multi-agency representatives to reduce length of stay, numbers of super-stranded patients and delayed transfers of care, to achieve performance targets related to discharge
Facilitate Trust's compliance with current legislation relating to discharge planning
