Job summary
The Tissue viability nurse specialist is an integral part of the Harm Free Care Team providing specialist, expert advice, treatment and education to support our clinical teams to deliver safe and compassionate care, reduce harm and improve patient experience.
This post is based at the RSCH/PRH/Newhaven sites. However, there may be a need for the post holder to be flexible at times to provide service delivery and advanced knowledge on all matters relating to tissue viability, across multiple hospital sites across the county of Sussex. There will be support and supervision to ensure the post holder is competent and confident to deliver the needs of the service across the differing sites.
Main duties of the job
Provide highly specialist clinical knowledge and holistic advice to clinical staff underpinned by theoretical and practical knowledge of a wide range of procedures and practices relating to traumatic, acute and chronic wound management. This will include the management of lower leg trauma and other tissue viability challenges.
Manage and prioritise referrals to the service, determining those requiring direct review and/or liaison with other agencies
Independently assess, plan, implement, monitor and evaluate care delivered to the tissue viability caseload of patients, as well as implementation of management plans created in collaboration with medical colleagues.
Support Matrons and other clinical staff using the AAR/ELR process for incidents to identify root causes and contributing factors for incidents, ensuring that this informs quality improvement work, and that appropriate actions and learning is shared across the Trust and wider community.
Advice on safeguarding escalation of admitted and acquired pressure ulcers.
Participate and initiate quality improvement works in relation to improve the Tissue Viability service.
Provide education programmes as required related to tissue viability as part of the Harm free Care plan within University Hospitals Sussex.
Make clinical judgements about patients and adjust treatment plans accordingly to achieve treatment targets.
