Job summary
We are seeking a Fixed Term Consultant General and Upper Gastrointestinal (UGI) Surgeon for a 6 month fixed term contract. The post holder will be expected to manage general surgical patients, working together with the team to optimise the array of tertiary services. The successful applicant will contribute to the overall performance of the General Surgery Service and will participate in an emergency on-call service for General Surgery on a 1:6 basis. The post holder will be responsible for regular out-patient clinics, endoscopy lists and theatre sessions at RSCH and PRH in a flexible manner.
Main duties of the job
This post will expand the consultant numbers and also provide sickness and shielding cover for the department. The expansion in consultant numbers has been conceived not only to better meet increasing demand but also to support tertiary level specialties and to reconfigure the acute service for our patients.
A new service model is being planned to include an emergency general surgery team 7 days a week. Acute and elective work components will be separated weekly and each consultant will work a rolling rota to deliver acute floor senior support, speciality ward rounds, on call cover and elective activity.
During the acute floor week the consultant will be in charge of the team delivering care to the Emergency Department, Ambulatory Care and CEPOD theatre lists.
During the ward round/on call weeks the consultants work in pairs, one with an UGI interest and one with a LGI interest. Daily consultant led ward rounds for each team ensure patient safety and facilitate efficient patient flows.

The clinical duties of the post include the following:
The Trust is funded for 16 General Surgical Consultants. They divide into two teams, 8 UGI and 8 LGI. Not all the current post holders contribute to the on call rota. This post will contribute to the on call work and acute rota. RSCH is the main site for general surgery in patients, emergency admissions and elective resectional work. All on call work is carried out on this site. Currently there are 12 persons contributing to the on call rota. This post will replace one of the locum UGI position and contribute to the resulting 1 in 6 on call rota. There is a 12 week rota cycle. Two of those weeks consist of ward cover and on call work, with the on call work split between the two persons on for that week. A third week is allocated to acute floor work acute attendances and CEPOD cover Mon-Fri. The remaining 9 weeks of the cycle are purely elective (see below). PRH is where day surgery/short stay elective general surgery procedures are carried out. Day time cover for the small number of in patients at PRH is provided in turn by a surgeon on site that day doing a theatre list or a clinic. Out of hours clinical concerns from the junior doctors regarding inpatients at PRH are escalated by phone to the on call registrar or consultant at RSCH and transferred to RSCH if necessary. Only in extremis would the RSCH on call consultant be required to travel to PRH out of hours Blocks of time for administration are allocated within the DCC allocation of the elective weeks. A whole block of time for administration is not allocated separately during the busy acute weeks but during on call afternoons, after the morning ward rounds, urgent administrative tasks can be attended to. The Trust has the required arrangements in place to ensure that all surgeons have an annual appraisal with a trained appraiser and supports surgeons going through the revalidation process.
The appointee is expected to be committed to the improvement of quality of clinical care in the Division and to participate in incident reporting, risk management and Divisional Clinical Governance Meetings
On call commitment
On call arrangements are being reviewed. Current on call frequency is 1:6. Following review and recruitment on call frequency is likely to reduce. On call duties are described earlier
The post holder accepts that they will also perform duties in occasional emergencies and unforeseen circumstances at the request of the appropriate consultant, in consultation, where practicable, with their colleagues, both senior and junior. While it has been agreed between the professions that they will perform such duties, the Secretary of State stresses that additional commitments arising under this sub-section are exceptions and you should not be required to undertake work of this kind for prolonged periods or on a regular basis.
Teaching and training
All consultants and specialist doctors are expected to take part in clinical postgraduate and undergraduate teaching.
Governance and audit
All consultants and specialist doctors are expected to take part in clinical audit and submit data to national audits as appropriate. Attendance at departmental monthly morbidity and mortality meetings and bimonthly governance meetings is mandatory for all consultants and speciality doctors.
Quality Improvement
It is a responsibility of all consultants and speciality doctors to identify areas for service improvement and to engage in quality improvement initiatives. The Trusts approach to QI is described later.
Research
Involvement in research is encouraged and opportunities can be discussed with your line manager on appointment.
Learning and Development
- Attend mandatory training updates as required.
- Undertake training as necessary in line with the development of the post and as agreed with the line manager as part of the personal development process.
