Job summary

At University Hospital Sussex, we are looking for 1 high calibre individual who can combine people management skills, analytical capabilities, and pragmatic management approaches to join our spinal service. We are looking to recruit 1 Band 6 spinal practitioners for 37.5 hours per week to cover maternity leave, to join our multi-professional team to support complex spinal surgery in line with GIRFT recommendations. This could be an exciting role of nurses or physiotherapists.

Main duties of the job

This post will support the emergency admission pathway and as part of the elective procedure process, from pre-assessment and post procedural care, ensuring all standards are met, promoting a patient centered and evidence based care pathway.

Job description

Job responsibilities

Clinical

  • Responsible and accountable for the coordination and delivery of patient care and to influence practice within the MDT.
  • Work on own initiative to coordinate the care of spinal patients, and support safe
  • The post holder will practice autonomously and support and empower all healthcare workers and patients through the provision of education, expert advice and clinical
  • Be highly skilled and competent in the comprehensive assessment of patients using theoretical knowledge and a wide range of experience to assess needs and administer care to support
  • Devise individual patient-led treatment plans using clinical reasoning, current evidence and a wide range of treatment
  • Facilitate Nurse/MDT-Led discharge for specific consultant based patient groups in line with locally agreed
  • Have an active role and facilitate spinal oncology and spinal MDTs liaise with other specialties involved in care of patients referred or under the care of spinal
  • Co-ordinate interventions which may include other disciplines through case management, multidisciplinary team meetings, integrated teams within the hospital and in the community.
  • Ensure all documentation is complete and accurate and that all information is stored and maintained appropriately by keeping databases and records

Clinical: Core Nursing/Health professional responsibilities

  • Responsible for the assessment, planning, implementation of care and
  • Interprets data with support from band 7 spinal practitioner, relevant to the patients condition and
  • identifies patient problems and needs with the patient, their carers, senior nursing staff and other staff as required.
  • Identify the priorities of care for patients and realistic goals for their care
  • Change care plans and goals according to evaluation and discussion with the multidisciplinary
  • Maintain accurate documentation to facilitate the continuity of care for patients transferred to other
  • Promote a safe clinical environment for all patients, visitors and
  • Maintain the patients respect, dignity and privacy and to act with kindness and courtesy to all members of the public.
  • Ensure practice is evidence based and is in line with local policies and Procedures
  • Liaise with SCI centers for patient with SCI
  • Liaise with oncology/haematology/palliative team for appropriate management of patient with MSCC

Neurosurgery Morning Meeting

  • Identify and document new Spine admissions
  • Document patients on White board waiting for transfer
  • Liaise with Consultant Spinal Surgeon on- call

Ward Round/Patient Care

  • Accompany Spine team on Ward Round
  • Liaise with
  • Set up MDTs to help reduce LOS.
  • Liaise with patients and families
  • Ensure all out-lying patients seen
  • Ensure ASIA scores and bowels regimes are completed
  • Liaise with out-lying teams (including nursing staff) and offering help and advice on the management of Spine patients e.g. collar care, TLSO, halo provide support to patients and other health care professionals regarding management of spinal orthotic devices.
  • Arrange for appropriate investigations as needed
  • Arrange appropriate follow-up for patients discharged from Emergency Department and wards.
  • Telephone patients at home with results / plan as it becomes available
  • To obtain copies of notes, X-rays & scans of patients to be repatriated from other Hospitals or abroad
  • To send copies of notes, X-rays & scans of patients to be repatriated to other hospitals or abroad
  • Liaise with Stoke Mandeville / SRC / other rehabilitation units for the care and safe transfer of patients with Spinal Cord Injury

Patients at Home/Other Hospitals

  • To liaise with surgeons, helping to arrange their theatre lists and informing the patients of the dates TCI
  • To liaise with spinal admission bookers to ensure theatre procedures and ITU/HDU beds are booked where appropriate
  • To explain admission procedure & NBM instructions to patients prior to admission
  • To help arrange transport for patients TCI when necessary
  • To be a point of contact for patients

Clinical operations

  • To liaise with Clinical Operations regarding daily discharges and finding beds for ER admissions as they occur
  • To liaise with Clinical Operations team re patient repatriations into & out of RSCH

Management

  • Contribute to service development and delivery and business planning with the ward or department manager and senior
  • Lead and attend departmental meetings and/ or multi-disciplinary meetings as
  • To be responsible for the maintenance of accurate and evaluative professional documentation, including initial assessment, treatment planning and intervention that demonstrate evaluation, analysis and clinical reasoning in the provision of patient
