Job summary
An exciting opportunity has arisen for an enthusiastic, innovative, motivated and dynamic Patient Flow and Complex Discharge Coordinator to be part of a regional project supporting improved discharge processes from the Royal Alexandra Children's Hospital.
This exciting position offers the opportunity for an individual to work autonomously with their own patient case load. This case load will consist of a wide range of patients, with multiple medical/surgical needs such as Long-Team Ventilation, those requiring home care packages. This is a full-time role which is currently funded for 1 year.
The successful candidate will lead and work as part of a multi-professional team to support the smooth running and co-ordination of complex discharges and avoid unnecessary readmissions. They will a point of contact for families and act as a liaison between them and their multiple medical/surgical professionals.
Main duties of the job
The Complex Discharge Coordinator will support discharges of all patients but with a specific focus on the longer-term patients and/or complex discharges and/or transition to adult services.
The role will improve the child and parents' experience and quality of care by reducing time spent in hospital.
The post holder will provide clinical expertise and decision making to facilitate timely discharges. They will work collaboratively with the MDT, parents and local hospital, community teams and external agencies to ensure a timely and effective discharge.
To ensure that the ongoing needs of the child are being appropriately met.
The successful candidate will work closely with all ward teams ensuring appropriate referrals are seen and made to local authorities and preventing unnecessary, avoidable delays for patient discharge.
