Job summary

Incident Manager

The Incident Manager supports the Head of Risk and Incidents and the Head of Patient Safety by leading on the development and the management of the Trust's Patient Safety system (currently largely in DCIQ, with some parts in Datix Web).

They will ensure that:

  • There is effective line management of the incident team.
  • The patient safety system is developed and maintained in line with the needs of the Trust.
  • Information held by the Trust on the patient safety system is accurate.
  • High quality reports are produced in line with business needs.
  • Incidents are escalated as appropriate and in a timely manner.

Led by the Head of Patient Safety, this role is largely a remote role, though there is a regular requirement to travel to Nexus House in Crawley and across the region to support the needs of the business.

We are looking for candidates with strong line management skills and recent experience of using DCIQ.

Main duties of the job

The Patient Safety Team works to ensure all elements of patient safety are managed efficiently and effectively, and that any trends and themes are triangulated and analysed. The team works collaboratively across the organisation to ensure that:

  • patient safety is routinely considered.
  • incidents are identified, reported, appropriately investigated and that lessons are learned; and
  • risks are identified, assessed and mitigated where possible

The team also has responsibility for statutory functions including the delivery of the Patient Safety Incident Response Framework (PSIRF), the Statutory Duty of Candour, the submission of patient safety incidents to the National Reporting and Learning System (NRLS) and learning from patient safety events (LFPSE.)

Staff management

  • Line management of four Incident and Risk Administrators within the Incident Team in accordance with appropriate Trust policies and procedures
  • Responsibility for all human resource functions within the Incident Team (e.g., recruitment, annual/sick leave and line management)

System management

  • The Trusts lead point of contact for all patient safety system queries
  • Responsibility for managing the access and use of the Trusts patient safety system (covering incidents, risk, patient experience, claims, safety alerts and CQC compliance) and all sub modules.
  • Responsibility for leading the design, coding and development of the patient safety system.
  • Liaising with RL Datix for system development and maintenance
  • Ensuring the patient safety system remains effective and up to date by being responsible for all upgrade requirements.
  • Delivery of the implementation of Datix iCloud
  • Maintaining the patient safety system as problems or new requirements are identified, including any bug fixing and any change requests
  • Fault logging, maintenance and intervention in conjunction with IT
  • Ensuring changes made to the patient safety system are documented and communicated to all relevant stakeholders via the appropriate change management process
  • Holding regular user group meetings to communicate changes to the system.

  • Reviewing existing policies and develop new policies when the need arises.
  • Escalating any areas of concern within appropriate timescales.
  • Remaining up to date with changes to patient safety to help facilitate an up-to-date incident module.
