Clinical Lead Practitioner - OPMH Liaison

Job summary
The post holder will be responsible for the development and clinical oversight of Older People Mental Health Liaison (OPMH Liaison), across Liaison Services in Queen Elizabeth Hospital and Princess Royal University Hospital, providing senior clinical leadership to the team.
The post holder will ensure that the OPMH Liaison services meet agreed objectives and effectively deliver a high-quality service, working in collaboration with Liaison Team Managers.
The post holder will be responsible for providing clinical supervision to OPMH Liaison clinicians and take responsibility for enhancing OPMH liaison services across the sites. The post holder may take on operational management of staff as agreed with local services and operational management.
The post holder will be required to travel between sites/ base themselves at different sites throughout the working week.
Main duties of the job
- Work alongside senior colleagues to provide clinical leadership to older people's liaison team.
- To support with the day-to-day management of the MDT.
- To ensure that effective communication is maintained.
- To be responsible for setting and maintaining standards of care.
- To liaise with community and inpatient teams regarding gatekeeping of patients to offer an alternative to hospital admission/expediate early discharge from an inpatient service.
- To be fully conversant with statutory and non-statutory services in Greenwich/Bexley/ Bromley and to promote effective liaison and operational links as appropriate.
- To actively participate in the development of Older People's Mental Health Liaison Services.
- To participate in the evaluation of the service in conjunction with the Senior Management Team and Quality and Audit Departments.
- To be responsible for the planning and facilitation of induction programmes for members of a multidisciplinary team.
- Direct clinical/management supervision of staff ensuring assessment of their professional responsibility and effectiveness.
