Job summary

The Source is a nurse led clinic based on the Horn Park Estate providing local residents with easy access to health and well-being services.

We are looking for a motivated, dynamic and interactive advanced care practitioner who can lead on the delivery and support the continued development of the service.

The postholder will need to be able to provide assessment and treatment to both adults and children over the age of 1.

The post holder will also be required to interact with both stakeholders and local residents to ensure that the services offered are meeting the needs to the local population.

Main duties of the job

The post holder will support the Service Lead in the development of the Source to meet the needs of the local populations, ensuring the highest possible standards of clinical practice.

The post holder will be expected to deputise /jointly lead on delegated operational and clinical tasks.

The service works in a collaborative way with the local neighbourhood and the post holder will need to be involved in public engagement and communications strategies to support service development based on feedback from the local residents.

The post holder will act as an expert clinical resource to team members facilitating high quality evidence-based practise and ensuring patients appropriately access out of hospital community health and social care service as appropriate

The post holder will be practising as a qualified ACP based at the Source (nurse led unit),this will encompass the skills of assessment, examination, diagnosis, and treatment within an agreed scope of practice for both adults and children aged over 1 year old.

  • To hold a MSc /PGDip in Advanced Clinical Practice or equivalent credentialed programme.
