Job summary

This is a exciting opportunity to develop your skills as a Community Mental Health Nurse working in a community mental health setting. You will promote and deliver evidence-based care for service users with mental health conditions in their own homes and within a variety of accommodation settings.

We are looking for a motivated, skilled, and experienced Nurse to join our Community Mental Health Team to offer clients under our Intensive case management for psychosis team, a high standard of care to support and enable recovery within the community.

You will be joining a Multi disciplinary team working closely with colleagues from OT, Psychology and Medicine specialties to improve the outcomes for our patients through planning effective interventions and care.

Main duties of the job

As a Community Mental Health Nurse, you will be responsible for a caseload of clients with enduring mental health needs, You will be required to plan their care which involves assessing and managing risks, delivering crisis interventions, offering support with Mental health difficulties as well as monitoring physical health and supporting with social needs. Working independently to manage your caseload, with access to the wider MDT team for support with interventions.

Additional duties include supporting the Depot Clinic, Clozapine clinic and Duty system. Training and induction will be provided.

Engaging with community resources and external agencies to aid the patients recovery.

Having knowledge of the legal requirements of the Mental Health Act, Care Act, Mental Capacity Act, Programme Approach, Deprivation of Liberty, Safeguarding Vulnerable Adults and Community Treatment Orders.

Administering and monitoring the side effects of medication according to NMC professional standards.
