Job summary
Rehabilitation Support Worker / Healthcare Assistant Band 3 Joint Emergency Team, Woolwich Full Time: 37.50 hours per week
This is an exciting opportunity to join our integrated team and support the collaboration between Oxleas NHS Foundation Trust, Royal Borough of Greenwich and Lewisham and Greenwich NHS Trust.
This post will work as a part of a multi-disciplinary team, including health and social care colleagues to meet the needs of clients referred to the Joint EmergencyTeam, ensuring the highest possible standards of practice.
This post will involve assisting the physiotherapy occupational therapy and nursing staff in delivering rehabilitation programme, treatment sessions, supporting activities of daily living and undertake clinical nursing activities.
Main duties of the job
- Assist patient with activities of daily living
- To work closely with the registered therapists, nurses and widerMDT.
- Support the implementation if care plans; including therapy plans.
- Engage patients in therapeutic activities, monitoring their physicaland mental state and progress.
- Undertake appropriately delegated clinical tasks once deemedcompetent i.e. Venepuncture - urinalyses - wound care - monitorblood pressure - temperature - pulse - respiratory rate - bloodsugar - (administer enoxaparin this is not essential but may berequired in the future band 3 HCA's are administering in thecommunity).
- To engage in supervision, reflecting on and modifying practicewhere indicated
- To promote independence and wellbeing for the patients to regaintheir previous level of independence as quickly as possible.
