Job summary

The post holder will be the Consultant Psychiatrist to the Greenwich West Anxiety, Depression, Personality Disorders and Trauma Team (ADAPT) with Oxleas NHS Founda?on Trust. This post hasbeen covered temporarily for the past 3 years by the Associate Specialist in the team 'ac?ng up'.

The post holder will carry no responsibility for inpatients but will be part of the bed management strategy within the Community Mental Health (CMH) Directorate. The Directorate takes a whole system approach to bed management with twice daily meetings between community and inpatientclinicians to ensure that beds are used in the most efficient manner. There is additional community support in the form of our admission avoidance team, the home treatment team and our crisis housebefore patients may need a hospital bed.

Main duties of the job

The ADAPT Pathway provides focused, therapeutic interventions to working age adults residing withinthe Greenwich Borough who require care and treatment for anxiety, depression, affective disorders(without psychosis), personality disorders and trauma.The service aims to be flexible and accessible. The pathways of care aim to offer individuals, choiceand involvement regarding their treatment, actively encouraging their participate on in the planningand implementation of their care. A personalised care plan will be devised in consultation with theservice user and their Carer/family where appropriate.The Greenwich ADAPT Team was formed as part of a reconfiguration of Community Mental HealthServices that occurred in September 2015. The reconfiguration of Community Services was part ofthe directorate's plan to ensure efficient delivery of mental health services whilst delivering highquality care.? The post holder will be responsible for the care and treatment of patients within the team.This will involve attending multi-disciplinary team meetings to discuss new referrals to theteam. The post holder will be expected to attend zoning meetings during the week to helpthe team prioritize contacts with patients, especially those service users who aredeteriorating and presenting with increasing risk.? The post holder will be expected to chair review/CPA meetings, offer crisis appointments topatients who are in the red zone and offer home visits and telephone advice to generalpractitioners and other professional who may be involved with the care of service users onthe ADAPT caseload.? The post holder will have timetabled outpatient clinics each week. On average in theMedical Clinics there are 4-6 patients and in the Assessment Clinics there are 2 patients ineach clinic. It is anticipated there will be 4-5 clinics, constituting direct clinical care eachweek.? There is protected time for administrative work, and the post holder will have access toappropriate digital/technological support to ensure timely communication with primarycare colleagues.? There may be some Mental health act related work including attending tribunals forpatients on community treatment orders (CTO), but this is generally unlikely within thispart of the community pathway.? The post holder will have supervision duties for the Trust Grade/ Trainees/ Specialty Doctorworking within the team. They will be expected to provide weekly supervision anddiscussion of the caseload and any relevant clinical issues.? The consultant will in collaboration with the team manager and senior colleagues reviewthe caseload to ensure that there is throughput in the team.? The consultant will be expected to support the team with any service or qualityimprovement activities, and will be supported to use their own SPA time for any personalresearch, teaching or Qi? There will from time to time be 'directorate' activities that the consultant will be requestedto support/lead on which may include complaint or serious incident investigation panels.? The CMH directorate aims to provide holistic care, with a focus on wellbeing and physicalhealth, for patients that we know are likely to die younger than their peers. The consultantwill be expected to lead on physical health care improvement within the team.? The consultant will be invited to be part of the monthly Greenwich medical peer group. Thebi-monthly directorate medical meetings and the quarterly Trust MAC.
