Primary Support Worker , Band 3 Full time, Permanent
This is an exciting and challenging new post in the Offender Healthcare Service at HMP YOI/ISIS. We are looking for Primary care Support Workers.
- The post holder will assist in the overall delivery of care as determined by the Healthcare co-ordinator team and / or Nurse in Charge; to support the management, lead and co-ordinate the implementation of the care coordination model within HMP YOI/ISIS.
- Ensuring prescribed care is carried out according to the agreed care plans in conjunction with the multi-disciplinary team. Support workers will support the Primary care team and contribute to the process of assessment and delivery of care to support ongoing development of innovative and integrated clinical practice.
- The post holder will support qualified staff in all activities relating to the delivery of high-quality patient-centred care. The post holder will work and act as a key member of the primary care team, supporting patients in achievement of core clinical outcomes.
You should possess good communication skills & the ability to work as part of a multidisciplinary team in a busy environment. A kind, caring, compassionate & professional attitude is essential.
Main duties of the job
- To support qualified staff in all activities relating to the delivery of high-quality patient centred care.
- Work and act as a key member of the team, supporting each patient in achievement of their planned outcomes.
- Set up and manage clinics for patients such as Phlebotomy, ECG, Vital signs, hearing screening test, health promotion/wellness clinic.
- To undertake an extended role such as offender-focused health promotion activities and programmes.
- Support Blood borne virus and Covid -19 screening activities.
- To work as part of the Integrated Healthcare Service, providing a range of quality services to offenders: This may include enhanced diagnostic assessment & treatment services and long-term conditions management.

IMPORTANT INFORMATION, PLEASE READ:
