Job summary

This Learning Disability Liaison Support Worker post has been created in Bexley to support Primary Care services to engage with hard-to-reach patients with learning disabilities in the community and support them to access annual health checks.

The Learning Disability Liaison Support Worker based at Queen Mary's Hospital, will work in the Community Learning Disability Team alongside the Complex Physical Health Nurses, and in close relationship with the Multi-disciplinary Team.

Home visits will be carried out where required to allow for person-centred care to be provided in the most convenient place for the service users, as well as frequent face to face Primary Care Service contacts.

The Learning Disability Liaison Support Worker will support primary care colleagues in their co-ordination of Annual Health Checks over a 12-month period and assist in the development of Health Action Plans.

Main duties of the job

  • To provide person-centred support to people with learning disabilities who require an annual health check to be provided by their Primary Care Service in Bexley and help them to liaise and connect with the appropriate service with support.
  • To improve people's experience and quality of an annual health check within the borough of Bexley.
  • To be part of the Community Learning Disability Nursing pathway and understand the underpinning guidance and operating procedures.
  • To signpost people with learning disabilities to services relevant to their physical health and wellbeing needs and provide details of this in an accessible format where required.
  • To assist people who may be eligible to access community services relevant to their physical and mental health needs.
  • To be informed and work in partnership with Bexley Learning Disability Team, Primary Care Services, and the Integrated Care Board in order to have a holistic understanding of the offer for people with learning disabilities within the community.
  • To escalate any issues which cannot be resolved at local level to the relevant Line Manager.

  • To attend and participate in an induction programme and all training opportunities relevant to the role.
  • To attend regular monthly supervision sessions and annual appraisals with the Senior Community Learning Disability Nurse.
  • To attend regular team meetings within the Oxleas Adult Learning Disability Service as needed. Plus any other appropriate meetings to ensure good team working with the MDT, PCNs and wider community teams.
