Job summary
We are a friendly hardworking dedicated multidisciplinary group of professionals in the Community Adult Physical Health Directorate who are looking for another Consultant to work with our Lead Consultant to further develop our community model to provide excellent care for our frail elderly patients.
If you possess expert senior decision-making skills, enjoy direct patient care, have outstanding communication skills and are able to lead, work and teach an enthusiastic team, this is the job for you.
Main duties of the job
The main responsibility of this post is to lead and develop the Frailty Virtual Ward and Urgent Community Response Service, however you will be required to work alongside our Lead Consultant and may be required to provide medical support across the range of community services and there will be opportunities to develop your special interest. If you enjoy a challenge and want to make a difference to the quality of care for frail patients at the interface of primary and acute secondary care, then please apply for this post.
