Job summary

We are a friendly hardworking dedicated multidisciplinary group of professionals in the Community Adult Physical Health Directorate who are looking for another Consultant to work with our Lead Consultant to further develop our community model to provide excellent care for our frail elderly patients.

If you possess expert senior decision-making skills, enjoy direct patient care, have outstanding communication skills and are able to lead, work and teach an enthusiastic team, this is the job for you.

Main duties of the job

The main responsibility of this post is to lead and develop the Frailty Virtual Ward and Urgent Community Response Service, however you will be required to work alongside our Lead Consultant and may be required to provide medical support across the range of community services and there will be opportunities to develop your special interest. If you enjoy a challenge and want to make a difference to the quality of care for frail patients at the interface of primary and acute secondary care, then please apply for this post. Job description

Job responsibilities

Oxleas provides a range of mental health and psychical health services across South East London and Forensic services within the South West. This post will support Community Services for the London Boroughs of Bexley and Greenwich. Meadow View is our 36 bedded Intermediate Care Unit at Queen Marys Hospital Sidcup. Patients are referred as step down patients from acute hospital for rehabilitation or step up referrals from GPs for short term multidisciplinary treatment.

Our Urgent Community Response service known as Bexley Rapid Response is a specialist urgent service for people who have suffered sudden acute deterioration in their health or functional needs. This multidisciplinary team receives acute referrals from LAS, 111, A & E, and GPs. The team will visit patients at home to carry out a comprehensive geriatric assessment, discussing the diagnosis and management plan with the Consultant and treating the patient at home to prevent unnecessary admissions to hospital.The Rapid Response Team are also responsible for the Bexley Frailty Virtual Wards which allows frail elderly people to receive hospital care at home.

The Virtual Wards prevent admissions by treating appropriate non-emergency medical problems at home and providing physiotherapy, occupational therapy and social services input where necessary. The Virtual Ward team also supports A & E and Inpatient Early Supportive discharges.The other community services provided by Oxleas include District Nursing, Specialist services such as COPD, Heart Failure, Cardiac Rehabilitation, Complex Wound Care, Lymphoedema Diabetes, Podiatry, Dietetics and Community Rehabilitation and Neurorehabilitation Teams. The Bexley Musculoskeletal (MSK) Service is based in Queen Marys Hospital, Sidcup, and the team includes Physiotherapists, Podiatrists, Hand Therapists and Pain Psychologists. Our colleagues in Older Peoples Mental Health Teams and Mental Health Crisis Teams also provide support to our physical health patients where required.

Clinical Duties of the Postholder:This job description is for a full-time 10 PA post holder.Should any applicant wish to undertake a part-time position, the clinical duties and job plan will be adjusted accordingly. There is no on-call responsibility attached to this post

The duties of the postholder will include:

Consultant leadership for the Bexley Rapid Response and Bexley Frailty Virtual Ward Team

Participation in Quality Assurance and Audit Projects Clinical supervision of Junior Medical staff Clinical review of all rapid response and virtual ward patients though direct assessment or via team board rounds and multidisciplinary meetings Clinical supervision of Advanced Nurse Practitioners in the Rapid Response and Virtual Ward teams 2 PA of supporting professional activities for audit work, service development, structured teaching and CPD No on- call commitments Annual leave cover will be provided by the Lead Consultant and the postholder is expected to cover for the Lead Consultant during their annual leave

The descriptions of the post cannot be exhaustive, and the post-holder may be required to provide Consultant frailty support across the wider community service provision following discussions with the Associate Director or Service Director, normally these will be broadly in line with the listed key responsibilities.

It is expected the postholder will liaise with other professionals within Oxleas, the wider community, Primary Care, Bexley Social Services, Voluntary Sector and the South East London Group of Hospitals, with regard to continuity of patient care.

The postholder will comply with the Trusts agreed policies and procedures to practice in line with the Royal College of Physicians Best Clinical Practice Guidelines and the General Medical Counsels Good Medical Practice Guidelines.

The postholder must have an appraisal and job plan review meeting yearly and fulfil the requirements for revalidation.

Clinical Governance and Quality Assurance: Expected contributions to Clinical Governance and responsibility for setting and monitoring standards (in accordance with local and national guidance, eg.
