Falls Pick Up Service - Urgent Community Response (UCR)

A great opportunity has arisen for an experienced highly specialist therapist to join our Urgent Community Response (UCR) Service.

The post holder will clinically lead in the further development of the UCR and falls pick up service, and lead in the training and education of the multi-disciplinary team.

Main duties of the job

The Highly Specialist therapist will work as a part of a multi-disciplinary team, including health and social services colleagues to meet the needs of clients referred to the Rapid Response Team, ensuring the highest possible standards of practice.

  • To lead on the Falls, Pick up Service/UCR developments.
  • Provide and deliver training packages to meet this developing service, this includes maintaining training records for team members.
  • To conduct specialist and generic assessments with the MDT for clients referred to UCR.
  • To organise transfer to intermediate care facilities (at home or intermediate care beds) as appropriate, Care Packages and emergency placements as required and onward referrals.
  • To provide advanced specialist therapy input to clients referred to the team.
  • To provide clinical leadership into the day-to-day delivery of the multidisciplinary team, ensuring the highest possible standards of practice.
  • To support the Team Manager in establishing and monitoring a robust clinical governance framework for the Rapid Response.
  • Your role will also include supervision of other therapists; supporting the Team Matron and team seniors in running the service; completing audits; and participating in service development activities and delivering and participating in the MDT & therapy in-service training programmes.
  • You will need excellent communication skills to liaise with the people with complex conditions and with other professionals.

  • Member of the Chartered Society of Physiotherapy/ RCOT

  • Post-registration experience of working at a senior level with adults/elderly
