Job summary

This is a fantastic opportunity for an experienced Band 5 physiotherapist to develop specialist skills and knowledge within the Greenwich Neuro Rehabilitation Team. We are seeking individuals who are caring and compassionate skilled clinicians who will provide clinical assessment, treatment and management of patients in community.

The Team provides opportunities for multi-disciplinary working and specialist training.

The successful candidate will be working across three pathways: Early Supported Discharge (Stroke), Neuro Rehabilitation, and Stroke Rehabilitation.

Main duties of the job

The Greenwich Neuro rehab team provides specialist neuro-rehabilitation to adults with an acquired neurological condition who are residents of The Royal Borough of Greenwich. The rehabilitation will be provided across various community settings (residential and nursing homes) and in clients' homes.o To provide neurology assessment and physiotherapy treatment to patients as part of the specialist Community Neurorehabilitation service.o To work with multidisciplinary team to manage those with neurology long term conditions.o To carryout holistic assessments and to communicate relevant medical information, assessment details and advice across agencies following guidelines for sharing information and informed consent.

Clinical Skills Maintain own Patient caseload Undertake patient assessment using standardised assessment tools and/or the single assessment process. Recommend appropriate interventions to promote independence and safety Monitor patient status, using clinical judgement and knowledge in regards to progressing and/ or making minor modifications to treatment programmes Maintain effective multidisciplinary communications in order to ensure that treatment goals are met and a high quality service is provided to patients Able to assess and treat a variety of neurological conditions with supervision as required Maintain accurate records including patient notes, message taking, minute taking and use of computerised data collection Ensure Manual handling knowledge and skills are updated and used To undertake a comprehensive assessment of patients, including those with diverse or complex presentations/multi-pathologies; use clinical reasoning skills and manual assessment techniques to provide an accurate diagnosis of their condition To use tact and persuasion to motivate patients, so enabling them to participate in their treatment programmes. To be responsible for organising and planning own caseload, to meet service and patient priorities, readjusting plans as situations change/arise. Deliver intervention with minimal or no immediate supervision from AHP colleagues, working as a lone practitioner Delegation and monitoring of clinical tasks to Therapy/Rehabilitation Assistants Ensure safe, appropriate and effective provision, installation and demonstration of equipment and further develop knowledge of minor/major adaptation provision and non stock equipment availability and use Contribute to meeting performance targets for the organisation To ensure confidentiality at all times, only releasing confidential information obtained during the course of employment to those acting in an official capacity in accordance with Trust policy and Caldicott guidance.

Management responsibilities Maintain high level of efficiency and effectiveness in work area to utilise resources to the full and report defects in equipment in a timely manner Ensure risk is managed for self, patients, colleagues and the work environment Develop the Specialist Community Neurorehabilitation by working proactively within the MDT Maintain throughput of casework and organise and prioritise own work tasks and activities, being flexible to the needs of the service Ensure new team members are adequately introduced to the Specialist Community Neurorehabilitation during induction by acting as a mentor and providing peer support Undertake a proactive role in the ongoing in service training programme for Therapy Assistants Maintain efficient day to day running of the Specialist Community Neurorehabilitation Ensure timely data collection and entry onto the Trusts data systems

Research Maintain continuing professional development by participating in regular in service training, supervision, reflective practice and attending study days and training, giving feedback to the team where appropriate to disseminate learning Maintain competency by attending statutory training and develop expertise in manually handling people and therapeutic manual handling Provide support and take a proactive role in the education and training of undergraduate and graduate AHP students and Nurses and Therapy assistants

Communication Communicate relevant medical/social information, assessment details, advice and recommendations across agencies following local and professional guidelines for sharing information and consent. This would include Consultants, GPs Social Services, other health care professionals and voluntary agencies Work in a co-ordinated, multi-disciplinary manner, communicating effectively with other team members and the wider clinical network involved in patient care Convey complex and sensitive information relating to patients using skill in situations where there may be barriers to communication i.e. use of interpreters, receptive/cognitive pathologies or excess noise Be able to adapt complicated and sensitive information into a simplified form to patients who may be seriously ill or have complex or long-term conditions Promote the Specialist Community Neurorehabilitation team by participating in informal talks for other agencies and professionals during education and training sessions Promote and encourage the ethos of rehabilitation with care assistants/carers/relatives through demonstration, example and support Maintain links with department, organisation and wider sector by attending meetings, seminars/case conferences etc. as deemed appropriate by AHP colleagues To be able to work effectively with patient and families requiring a language interpreter and ensure effective treatment for all patient regardless of their spoken language or communication status.Physical, Emotional and Mental Effort Be able to operate as a lone worker in the community setting Be able to cope with occasional, potential stressful, upsetting or emotional situations in an empathetic manner, such as patients suffering from illnesses or pain states that may lead to anxiety and aggressive behaviour and be competent in escalation of situations beyond scope to a senior practitioner. Comply with the Trusts Manual Handling Policy and local therapeutic handling guidelines and develop high level manual handling skills when carrying out assessments and treatments, with moderate physical effort throughout the day. Work in an environment where the work patterns may be disrupted by frequent demands from patients, clinical staff, students and administrative support staff and respond to any emergency situations arising. On-call / Unsocial hours The service has elements that run from 08:00 to 17:00. Currently, the service operates Monday to Friday, but soon it will transition to a seven-day working schedule and the post holder will be required to participate in weekend working on a rota basis.
