Highly Specialised Speech & Language Therapist, Band 7 Permanent
The Forensic Intellectual and Neurodevelopmental Disabilities (FIND) team is seeking to appoint a motivated Speech and Language therapist to support the new and innovative FIND in-reach service under the auspices of South London Partnership's (SLP) Learning Disability and Autism pathway. The SLT will support mainstream wards across the SLP forensic estate to provide consistently excellent, person-centred care to people with intellectual (learning) disability or autism and to those who fall just outside these diagnostic categories but whose recovery and discharge would be accelerated by specialist input.
The new FIND In-Reach Team will increase the capability of mainstream wards to manage people with neurodevelopmental conditions and reduce the need for people to be transferred to out-of-area hospital beds, helping SLP to achieve the key aims of the Transforming Care agenda.

Main duties of the job
To work within the Forensic and Offender Healthcare Services Directorate in providing support to individuals with intellectual disability and/or autism who are admitted to secure (forensic) wards within the three NHS Trusts that collaborate to form the SLP.
As an expert practitioner, and source of specialist advice to clinicians, allied health care professionals, service users and their carers, the postholder will help to develop a specialist SLT practice-based clinical model of care to secure forensic mainstream wards across the SLP.
The post holder will be required to communicate and provide highly complex information to a wide range of internal and external stakeholders.
- To review and reflect on own practice and performance through effective use of professional and operational supervision and appraisal.
- To participate as part of the MDT within the FIND Community Team
- To provide training across a range of agencies on issues pertaining to intellectual disability and autism.
- To plan and organise complex activities or programmes, requiring formulation, adjustment.
- To plan specialised SLT service provision and/or specialised health and education programmes.
- To develop protocols for specialist area, impact on other disciplines.
- To manage/support a defined caseload of complex service users who present as high risk and will have demonstrated this through the nature and gravity of their offending

- Provide specialist knowledge on forensic intellectual disability and/or autism across all forensic secure wards within the south London boroughs through active follow-up of a defined caseload as well as providing expert advice and consultation to multi-disciplinary teams, eg CMHTs, Community LD teams, Community Forensic mental health teams, courts, police liaison meetings, probation and mental health hostels.
- Assess, develop and implement care packages for service users within the framework of the CPA/CTR in conjunction with other members of the MDT.
- The postholder will ensure that people with intellectual disability or autism are not disadvantaged when admitted to mainstream wards, and any additional needs or adaptations to care are fully assessed, understood, and catered for. This includes communication support and reasonable adjustments to support service users mental capacity and autonomy. The postholder will also have a role in improving access to physical healthcare, eg the learning disability annual health check.
- Provide direct clinical Speech and Language assessments and interventions with people with intellectual disability or autism and those just below the thresholds for these diagnoses admitted to mainstream wards, eg specialist assessment, communication support, psychoeducation.
- Provide consultation and advice to ward teams in specific cases, including SLT considerations, access to physical healthcare, providing reasonable adjustments, signpost to specialist services to maximise communication skills for service users to elicit a greater degree of independence and participation.
- Provide enhanced provision around communication include the development of communication passports, Talking Mats, assessments for assistive technology needs
- To develop effective systems and interfaces with community health services ensuring continuity of care from secure services to the community and to actively promote joint working across professional and organisational boundaries.
- Provide training and education to multi-disciplinary staff teams to improve staff confidence and self-efficacy on people with intellectual disability or Autism;
- Feedback to the LDA Pathway and strengthening links between specialist in-patient, step-down, and community components of the pathway;
- To contribute to the triaging of ASD referrals.
- To operate as sole lead working internally and externally to deliver the project aims, initiatives and services to time and in a cost-effective way.
- To support SLT students on placement and liaise with the head of profession for practice development and academic pathway.
