Due to additional funding, the Greenwich Community Long-term conditions Teams are expanding the Psychology provision. This is an exciting opportunity to join our forward-thinking and dedicated multi-disciplinary team of highly specialist nurses, dietitians, podiatristsand medical Consultants.
We are looking for a highly specialist clinical/counselling Psychologist, preferably with experience in physical health, Diabetes and/or Long-Term Conditions, to be part of our multidisciplinary teams.
This is a varied post and an exciting opportunity to work across a number of long-term conditions including Diabetes, COPD and Cardiac teams in a community-based setting in the borough of Greenwich. A significant proportion of the post will be with the Diabetes Team. Greenwich Community Diabetes Team works closely with primary and secondary care and runs clinics throughout the borough of Greenwich.
The postholder(s) will be joining a dynamic and friendly network of psychologists in physical health in the Adult Community Physical Health Services Directorate. There are regular opportunities for CPD, networking and coming together with other psychologists working in physical health for collegiate support. Clinical supervision is provided from within the directorate by a Consultant Clinical Psychologist, Directorate Head of Psychological Therapies.

Main duties of the job
Goldie Leigh Hospital is the main base but work at various clinics around the borough as well as home visits within the borough is part of the role.
As a senior clinical member of the Long-term conditions (LTC) Teams the post holder(s) willbe responsible for the autonomous provision of a highly specialised psychological therapy service for patients with diabetes/other LTCs.
The post holder(s) will develop and lead the service ensuring routine collection, review and feedback of outcome data, and that activity targets are met. They will be accountable for the direct provision of a systematic, specialist clinical/counselling psychology service ensuring comprehensive, multidisciplinary assessments and a range of psychological interventions appropriate to individuals with diabetes/ other LTCs. As well as direct work, the postholder(s) will also provide consultation and reflective practice for the teams and professionals involved in the patient's care and act as a psychological resource for the teams.

Clinical:
- To provide specialist psychological assessments of Community Long-term Conditions clients and other people referred to the team based upon the appropriate use, interpretation and integration of complex data from a variety of sources including psychological and neuropsychological tests, self-report measures, rating scales, direct and indirect structured observations and semi-structured interviews with clients, family members and others involved in the clients care.
- To formulate and implement plans for the formal psychological treatment and/or management of a clients problems, based upon an appropriate conceptual framework of the clients problems, and employing methods based upon evidence of efficacy, across the full range of care settings.
- To be responsible for implementing a range of psychological interventions for individuals, carers, families and groups, within and across the service, and other teams where necessary or appropriate and in synthesis, adjusting and refining psychological formulations drawing upon different explanatory models and maintaining a number of provisional hypotheses.
- To evaluate and make decisions about treatment options taking into account both theoretical and therapeutic models and highly complex factors concerning historical and developmental processes that have shaped the individual, family or group.
- To exercise autonomous professional responsibility for the assessment, treatment and discharge of clients whose problems are managed by psychologically based standard care plans.
- To provide specialist psychological advice guidance and consultation to other professionals contributing directly to clients formulation, diagnosis and treatment plan.
- To contribute directly and indirectly to a psychologically based framework of understanding and care to the benefit of all clients of the service, across all settings and agencies serving the client group.

Leadership, Teaching, training and supervision 1. To receive regular clinical professional supervision from a senior clinical psychologist and, where appropriate, other senior professional colleagues.
- To develop skills in the area of professional post-graduate teaching, training and supervision and to provide supervision to other MDT staffs psychological work as appropriate.
- To provide professional and clinical supervision of Clinical/Counselling Psychologists, Trainee and Assistant Clinical/Counselling Psychologists and, as appropriate, to contribute to the supervision of individual pieces of work for Trainee Clinical/Counselling Psychologists.
- To contribute to the pre- and post-qualification teaching of clinical and/or counselling psychology, and other relevant healthcare staff, as appropriate.
- To provide advice, consultation and training to staff working with the client group across a range of agencies and settings, where appropriate.
Management, recruitment, policy and service development
- To contribute to the development, evaluation and monitoring of the teams operational policies and services, through the deployment of professional skills in research, service evaluation and audit.
- To advise both service and professional management on those aspects of the service where psychological and/or organisational matters need addressing.
3. To be involved, as appropriate, in the shortlisting and interviewing of assistant/graduate psychologists
- To participate in recording information as per local policy so as to provide information about referrals and activity as needed
- To manage the workloads of Clinical/Counselling Psychologists, Assistant and Trainee Clinical Psychologists, within the framework of the team/services policies and procedures, where applicable.
Research and service evaluation
- To take the psychology lead, as a senior clinician, in the evaluation, monitoring and development of the teams operational policies, through the deployment of professional skills in research, service evaluation and audit and ensuring incorporation of psychological frameworks for understanding and provision of high quality care.
- To utilise theory, evidence-based literature and research to support evidence based practice in individual work and work with other team members.
- To undertake appropriate research and provide research advice to other staff undertaking research.
- To initiate project management, including complex audit and service evaluation, with colleagues within and across the service to help develop and improve services to clients and their families.
Communication
- Establish and maintain communication with people, both internally and externally to Oxleas NHS Foundation Trust
