Job summary
Bexley Complex Case Team are looking for an Highly Specialist Physiotherapist to join the team. The successful post holder will work clinically within the team, providing clinical input and leadership within the multi-professional team. The post holder will be an expert Physiotherapist with extensive clinical experience in the management of patients.
The service will operate Monday to Friday 9am - 5pm
Main duties of the job
The Complex Case Team provide interventions into the Care Homes in Bexley. Our aim is to support residents in the care homes to avoid unnecessary hospital admissions. The team facilitate MDTs with the care homes which include community services, mental health and GP's to provide interventions required to support the patients in the care homes.
The Physiotherapist will work as a part of the multi-disciplinary Complex Case Team to meet the needs of patients referred to the team, ensuring the highest possible standards of practice.
The Complex Case Team provides rehabilitation, including Occupational Therapy, Physiotherapy and Nursing to adult clients living in the borough of Bexley, across various community settings and in their own homes. The physiotherapist will work as part of the multidisciplinary Integrated Team to meet the rehabilitation needs of the service users and their carers, enhancing independence, safety and quality of life, maintaining people in their own home environments.
