Job summary

Community COPD Nurse/Physiotherapist, Band 6 Permanent - Full Time - 37.5 hours per week

7 day working

We are pleased to offer an exciting role to work within a large multi-disciplinary team in the Greenwich COPD Service. The team provide acute support and chronic disease management of patients diagnosed with COPD with a community setting. Within the team we provide a 7 day hospital admission avoidance service, a diagnostic spirometry service for COPD, manage and review patient on home oxygen therapy and deliver Pulmonary Rehabilitation.

The role will include working as part of the team to providing holistic assessment and treatment tailored to the patients' individuals needs, and education about the condition. This will include making onward on referrals to other agencies as appropriate

You will need to be a versatile, adaptable team player who has good organisational skills. You will be required to use multiple computer systems therefore a good knowledge of IT is essential.

We offer regular in-service training and clinical supervision as well as the opportunity to be involved in student education, training and supervision of our newly qualified staff. There is also support and development through regular appraisals, a personal development plan and regular formal supervision.

Main duties of the job

The post holder will be part of a Primary Care led and patient-focused COPD service, delivering evidence based patient care, case finding and promoting health through a proactive approach to care management.

The role will include working as part of the team to providing holistic assessment and treatment tailored to the patients' individuals needs, and education about the condition. This will include making onward on referrals to other agencies as appropriate

The post holder will be required to work in a multidisciplinary way and work jointly with the other team members to ensure that all care needs are met. The team work closely alongside secondary care, meeting weekly with specialist nurses and a respiratory consultant for clinical support.

The post holder is an autonomous practitioner and will assess, plan, implement and evaluate patient care and also participate in the training of patients, relatives, carers, students and other health care professionals

Management

  • Responsible for a caseload of patients, using recognised criteria and guidelines.
  • Responsible for direct referrals to other disciplines as appropriate.
  • Co-ordinate and facilitate the care of patients, preventing fragmentation duplication and delays in care.
  • To anticipate present and future care needs of patients and develop plans to meet these needs, with particular focus on avoiding unnecessary hospital admissions.
  • To adhere to all Trust policies procedures and standards of practice.
