Job summary

The Greenwich Integrated Community Care and Rehabilitation team (CAR) is seeking an enthusiastic occupational therapist to join our team. The team comprises of Occupational Therapists, Physiotherapists, nurses, social workers, Care managers, assessment officers and therapy assistants. The post holder will provide functional rehabilitation to patients in the discharge to assess unit working with the wider MDT to ensure safe discharge from the D2A units and also will will work with Continuing Healthcare (CHC) Adults services, Health and social care partners to carry out timely and proportionate Occupational Therapy assessments.

You will need to be a versatile, adaptable team player who has good organisational skills. You will be required to use multiple computer systems therefore a good knowledge of IT is essential.

Main duties of the job

This will be a split post providing Occupational Therapy to clients in the Discharge to Assess (D2A) Unit and working with Continuing Healthcare (CHC) Adults services.

  • The post holder will provide functional rehabilitation to patients in the discharge to assess unit working with the wider MDT to ensure safe discharge from the D2A units.
  • The post holder will work with Continuing Healthcare (CHC) Adults services, Health and social care partners to carry out timely and proportionate Occupational Therapy assessments.
  • To provide Highly specialist Occupational Therapy service for inpatients in the discharge to assess units
  • To work with the wider MDT to discharge plan to ensure safe and pro-active discharges from D2A pathway
  • To practice within the multidisciplinary team as an autonomous practitioner, ensuring the highest possible standards of practice
  • Through assessment recommendations, plan how to meet CHC needs using the full range of available resources in the Local system / National guidance, with the aim of supporting the resident / carer to optimise their level of functioning / safety using strengths-based approaches.

  • To undertake specialised OT assessments in the D2A units and clients home using evidence-based tools and approaches to identify physical, cognitive and psychosocial skills and the impact of impairments of functional (ADL) tasks
  • To contribute in ensuring care packages are proportionate, recommending relevant assistive equipment / adaptations,assistive technology, manual handling techniques
  • To demonstrate highly specialist knowledge of CHC casework and related government guidance / legislation across health, social care and the voluntary sector.
  • To participate in regular MDTs to ensure discharge plans are co-ordinated
  • To be highly effective in working with clients who have conditions which involve multiple presentations and contraindications.
  • To carry out assessments where the situation is deemed to be significantly complex and wheremultiple high-risk factors may have been identified or anticipated due to medical condition, disability, environment, behaviour, social dynamics and safeguarding situations.
  • Promoting the use of specialist Occupational therapy knowledge in the provision of cost effective and cost-efficient assessment, intervention, and support for long term care needs.
  • To participate in joint visits with colleagues from D2A teams and CHC team Nurse facilitate good communication with clients, their carers / care agencies where there may be challenging / contentious dynamics and where timely or time limited interventions may be required.
  • To promote strength-based practice and positive change in CHC Adults lives and D2A clients by using Occupational therapy approaches, models and tools with the aim of helping them to participate in their health and wellbeing.
  • To be responsible and accountable for management and implementation of client rehabilitation programmes in the community
  • To be responsible for obtaining client and/or carer consent prior to Occupational Therapy intervention in accordance with professional guidelines
  • To ensure management of clinical risk within own caseload
  • To maintain up-to-date clinical written records using relevant computer systems including MOSIAC and RIO in accordance with local and professional standards; and also writing comprehensive discharge and progress reports
