Job summary

The Greenwich Memory Service provides assessment, diagnosis, treatment, support and education to those that are referred via their GP. The service is multi-disciplinary and works closely to deliver a service that meets the service users and carers needs.

This particular post will provide specialist OT services to the Memory Service incorporating assessments, individual and group work, link work, providing support to carers, close working within the MDT and supporting the services development.

You will provide occupational therapy input to clients referred to GMS. This will include providing an assessment and provision of both non-complex and complex equipment to ensure that the client is safe and to maintain independence in their own home. The post holder will be required to work in a multidisciplinary way and work jointly with the other team members to ensure that all care needs are met. They will be required to carry out an holistic assessment of the client within their own homes and devise and contribute to a multidisciplinary care plan. This will include making onward on referrals to other agencies as appropriate.

You will need to be a versatile, adaptable team player who has good organisational skills. You will be required to use multiple computer systems therefore a good knowledge of IT is essential.
