Job summary

The Forensic Intellectual and Neurodevelopmental Disabilities (FIND) team is seeking to appoint a motivated RNLD/RMN to support the new and innovative FIND in-reach service under the auspices of South London Partnership's (SLP) Learning Disability and Autism pathway. The RNLD/RMN will support mainstream wards across the SLP forensic estate to provide consistently excellent, person-centred care to people with intellectual (learning) disability or autism and to those who fall just outside these diagnostic categories but whose recovery and discharge would be accelerated by specialist input.

Together with the FIND Community Team, and concurrent with the development of secure ward for people with an intellectual disability and community step-down provision, the postholder will significantly enhance the offer to people with intellectual disability or autism using SLP forensic services. The new FIND In-Reach Team will increase the capability of mainstream wards to manage people with neurodevelopmental conditions and reduce the need for people to be transferred to out-of-area hospital beds, helping SLP to achieve the key aims of the Transforming Care agenda.

Main duties of the job

  • Provide specialist knowledge on forensic intellectual disability and/or autism across all forensic secure wards within the south London boroughs through active follow-up of a defined caseload as well as providing expert advice and consultation to multi-disciplinary teams, eg CMHTs, Community LD teams, Community Forensic mental health teams, courts, police liaison meetings, probation and mental health hostels.
  • Assess, develop and implement care packages for service users within the framework of the CPA/CTR in conjunction with other members of the MDT.
  • To improve access to physical healthcare, eg the learning disability annual health check.
  • Provide direct clinical assessments and interventions with people with intellectual disability or autism and those just below the thresholds for these diagnoses admitted to mainstream wards, eg specialist assessment, communication support, psychoeducation.
