We have an exciting opportunity for a qualified healthcare professional (nursing or AHP/HCPC registered) to join for our newly commissioned Lymphoedema service in Greenwich. We have been working closely with commissioners, primary care, and local stakeholders to commission the Greenwich Lymphoedema team. The proposal was developed to ensure parity with services available in all other South East London boroughs where currently Greenwich is the only borough without a lymphoedema service.
The post holder will be based in Plumstead Health Centre and will be expected to travel within the boroughs of Greenwich and Bexley.
The role will require a combination of working within the team base, in patients own homes, clinic settings across the Borough of Greenwich. There will be a requirement to participate in local networks including: The Pan-London Lymphoedema Community of Practice and The South East London Cancer Alliance Lymphoedema Expert Reference Group.

You will need to be a versatile, adaptable team player who has good organisational skills. You will be required to use multiple computer systems therefore a good knowledge of IT is essential.
Main duties of the job
To undertake clinical role as an Lymphoedema Specialist Practitioner including taking a lead role in assessment, diagnosis and treatment of patients on the Lymphoedema caseload.
To assess patients, to develop, plan, implement and monitor care. To provide highly specialist lymphoedema advice and treatment. Maintains appropriate clinical and non-clinical records.
To provide specialist education and training to other staff, students and/or patients/clients.
To support the Team manager in developing the service
Exercise leadership and judgement to ensure good clinical practice and high standards of care are maintained in order to meet service demand.

- To be a clinical specialist in all aspects of Lymphoedema and will provide care and treatment to patients in their own homes and in clinics
- To provide a fully comprehensive Lymphoedema service, including initial assessment, intensive treatment packages and follow-up sessions
- To deliver an efficient and effective service for patients according to best and evidence-based practice
- To raise awareness of the Lymphoedema condition and the Lymphoedema service
- Adhere to excellent standards of care within the Lymphoedema Service
- Participation in audit, research, education and attend relevant education courses/conferences to continually update knowledge in Lymphoedema care and management
- Teach other health professional and carers how to manage Lymphoedema
- Develop and maintain links with Community Matrons, Long Term Conditions Teams, Specialist Teams and General Practitioners, developing clear protocols, procedures and documentation ensuring the patient is kept at the centre of the care plan.
- Deliver high standards of Nursing documentation and communication within the team and with other professional referrals, building on best practice.
- Provide specialist Lymphoedema advice to patients, carers and relatives based on clinical evidence when required.
- Participate in MDT meetings and family conferences as required.
- Maintain up-to-date clinical written records and computerised records in accordance with local and professional standards; and also writing comprehensive reports on patients as required, inputting onto RiO.
- To provide line management to Band 6 Lymphoedema specialist nurses including supervision, performance management and appraisal.
- To deputise in the absence of the Clinical Lead for Lymphoedema and Complex Wound Care teams where appropriate
