Job summary

Community and Memory Service Improvement Manager, Band 8a Fixed Term Part Time : 22.5 hours per week

An opportunity has arisen for an experienced manager who is passionate about excellence in mental health care for older people. You will be responsible for the older peoples community mental health service, memory service and care home team.

Main duties of the job

You will need to be able to develop excellent working relationships across our new service line as well as influencing the older peoples mental health agenda across our developing Primary Care networks and third sector organisations.

This is a fixed term, part-time role, and will be based at Bexleyheath Centre

Job responsibilities

CORE OBJECTIVES:

  1. Implement the joint service framework for the locality, including continuous development of the service, which will include:
  • Delivering changes as we move away from integrated CPA and care management systems towards intervention based care
  • contributing to and implementing service objectives and standards
  • contributing to and implementing agreed joint policies and procedures
  • implementing integrated human resources systems; including appraisal, supervision, manpower planning, training and recruitment etc.
  • ensuring an integrated response to risk assessment, management and critical

incident reporting

  • financial management responsibility for budget and annual planning for the community mental health teams and memory service
  • ensure joint information systems are kept up to date, utilising management and clinical information to inform service management of development
  • manage the performance of the service as detailed through contracts, agreed national and local performance standards and be responsible for ongoing audit of service.
  • any other duties commensurate with the band.

2. In partnership with the Community Mental Health Directorate Management Team, deliver an integrated quality system meeting the requirements of both Health Trust and Local Authority, to include: Requirements of clinical governance

  • utilisation of evidence-based practice

  • implement service user and carer involvement strategies
  • ensure that service availability and accessibility is equitable and non-discriminatory across the borough

  1. Implement the service development objectives for the locality, to include:
    • ensure audits are completed as a method of service planning
    • develop and implement project plans
    • review and monitor progress
    • Work with partner departments and external agencies in order to deliver the whole system requirements/components required for a comprehensive community service.
    • Ensure data is collected to inform future service planning.
    • Provide effective leadership in managing change within the community sub directorate.
    • Work collaboratively across agencies and service areas in the development of the service.
    • Establish appropriate staff and team meetings.

  1. Deliver effective and efficient operational management of the service, including:
    • Effective co-ordination and leadership of the multi-disciplinary management team.
    • Direct management supervision and support of team managers, office manager and lead therapies staff within the locality.
    • Establish and continuously evolve effective operational systems within the locality, e.g., screen and allocate referrals; caseload management; duty rostering; acute admission pathways; admin. systems, etc.
    • agree, monitor, and review localised service objectives
    • respond to complaints and critical incidents as per policies and procedures
    • problem solve day to day issues connected with service delivery

  1. Implement effective partnership working including:
    • lead the development of user involvement
    • lead on carer involvement
    • lead on liaison with local GP practices
    • deliver an integrated joint service culture
    • Ensure effective communication and integrated care provision between community and acute services.
    • ensure effective liaison with other provide agencies, i.e., housing, addiction services, employment and day services, probation and police, forensic services, children and families, local CHC patients forum, etc.
    • Sustain effective working partnerships with other sectors within Bromley, Bexley and Greenwich

  1. Undertake personal development to include:
    • proactive participation in management supervision, annual objective setting and PDR/appraisal
    • participate in training and development opportunities
    • maintain knowledge of best practice within mental health services
