Bromley PCP - Band 6 Community Psychiatric Nurse

We are looking for a motivated, skilled, and experienced Community Psychiatric Nurse to join Bromley PCP.

Bromley PCP is the single point of access to secondary mental health services in Bromley. We receive referrals from various sources but principally primary care sources including GPs and the London Borough of Bromley. This is an exciting opportunity to be involved with the development of the service and requires a commitment to integrated working.

We are looking for a health care professional with exceptional organisational and communication skills

The post holder will be responsible for undertakingcomprehensive telephone and face-to-face assessments of referred patients to identify their needs; they may have to hold a caseload for a short brief -intervention period.

The post holder will be workingin a supportive and diverseteam, where there areopportunities for personal development.

The post holder will be a mental health professional registered with NMC with significant post qualification experience, working either in a single point of access, primary care, or community setting.

Main duties of the job

A Community Psychiatric Nurse will act as an effective member of an integrated Team, with responsibility for face-to-face and telephone assessments of incoming referrals from various sources within the borough of Bromley but principally referrals from GP practices and social services teams in the borough.

Bromley Primary Care Plus (PCP) is the point of access to secondary mental health care service in Bromley. There is an ongoing integration work with third sector and other organisations in line with the NHS 5-year Plan

To use standardised and assessment and screening tools to gather baseline information and make a formulation. Therefore excellent assessment and formulation skills are essential for this role.

To be able to follow instruction but work autonomously, with good attention to detail.

To maintain appropriate written and computerised records.

Provide supervision, mentorship & leadership for junior staff within the Bromley Primary Care Plus Team and Student Nurses, as appropriate.

To be able to contribute to the delivery of peer and student supervision and participate in the development and implementation of team protocols and the provision of information for services users and carers.

Maintaining service user confidentiality at all times.
