Job summary

The post has been created as part of the new service specifications and contract for April 2022, when Oxleas NHS Trust takes over as prime provider for the Wandsworth Prison Service.

The post holder will be a Consultant Forensic Psychiatrist based at HMP Wandsworth In-Reach Team.

HMP Wandsworth is one of the largest and most over-crowded prisons in Western Europe and accommodates up to 1668 prisoners. It is a category B resettlement prison.

Mental health service: At HMP Wandsworth we aim to detect mental health problems, refer urgent cases for transfer to psychiatric hospital, manage stable patients within the prison, and help plan for their discharge. CPAs are held every 6 months.

The Addison Unit is a 12 bedded mental health unit set within HMP Wandsworth. The unit consists of a Multi-Professional Team comprised of medical, nursing, OT and psychology staff. Care provision is delivered jointly with prison officers who manage the operational environment. The ward consists of a cohesive and multi-disciplinary team that utilizes a person-centred and recovery-focused model. The team demonstrates a wealth of experience and learning and development is viewed as paramount in supporting clinicians to become more informed and competent practitioners.

Main duties of the job

The post holder will be eligible for inclusion in the Specialist Register in Forensic Psychiatry or General Adult Psychiatry.

The position will provide Consultant leadership for the In-Reach team at HMP Wandsworth. In addition, the post holder would be expected to contribute to an aspect of quality assurance.

The post holder will be supported by a second consultant (to be recruited to and covering the In-Patient Unit), ST4-6 higher Forensic Trainee and 2 Speciality Doctors.

The post includes an interesting and varied range of clinical responsibilities for prisoners with a wide range of clinical conditions, including acute and chronic mental illness, personality disorder and ADHD. The in-reach team holds a caseload and within this caseload, number of patients would be subject to CPA management. New assessments are normally slotted into prearranged out-patient Clinics if the new assessments are generated from the referral system. All referrals are triaged by the In-Reach Team. On rare occasions, assessments are requested as an emergency from the wings or segregation unit. This is variable but on average couple of times a week. Whilst there is flexibility in allocation of individual responsibilities, the overarching model is one of integrated working across the mental health multidisciplinary teams.

There is protected time for administrative work, as well as protected time to deal with mental health act related work including attending tribunals for patients in the service and completing necessary Mental Health Act paper work relating to patients on community treatment orders. It is expected that the post holder will prepare medical reports for managers hearings and First Tier Tribunals as well as attend managers and tribunal hearings.
